Job Overview:Type: Regular, part timePay: $67 per hourSchedule: 10.5 hours (1 full day and 1 half day) per week. Specific days and hours will be determined based on clients and OT's availability from Monday to Friday between 9-5.Program Summary:You will be working in an exciting new DOHMH program for older adults over 55 called Living in Place. The premise of the program is to assist older adults with serious mental illness to age in place. All Living in Place clients are stabilized and in treatment and have been referred to this program to encourage independent living. This will be achieved by providing critical time interventions and other support services. Social determinants of health specific to the aging population such as economic stability, social activities, access to health care, nutritional food, and safe home environments will be addressed. The work will be delivered through an interdisciplinary team consisting of a social worker, wellness specialist, occupational therapist, and peer. Most of the interventions will occur in supported housing and private residences.Job Summary:As the Occupational Therapist, your main responsibilities will be to travel to your clients residence (typically in supported housing and/or private residences) to assess older adults' daily living/instrumental activities of daily living skills and fall risk/ prevention. You will collaborate with a team of social workers, wellness specialists, and peers to provide holistic care to clients to ensure they are safe and supported - all initial assessments will be performed as a team in the client's home.Qualifications:Must be licensed as an occupational therapist in New York StateExperience performing standardized ADL assessmentsCapable to travel throughout the 5 boroughs of ManhattanExperience working in community settings and/or older adults' homes.Able to evaluate need for fall prevention aid and assess for fall riskExcellent verbal, phone, and computer skillsExcellent communication and engagement skillsFamiliarity with Electronic Health Records a plusMust be a team player and work with multi-disciplinary staffBilingual English/ Spanish a plusResponsibilities:Implement one of the following assessments: the Lawton-Brody Instrumental Activities of DailyLiving and/or Katz Index of Independence in Activities of Daily Living.Advise on daily/ instrumental living skills improvements based on assessmentsAssist with determining fall prevention aidsFacilitate referrals and successful service connections to maintain safety in the homeExperience working in community settings and/or older adults' homes.Share resources that help clients maintain independence in the home.Collaborate with other program staffUtilize engagement skills for older adultsWork in this program occurs mostly outside the 4 walls of the agencyCapable to travelOther duties as assigned by the supervisor.Essential Physical Job Functions:Ability to walk on sidewalks, up and down stairs, and throughout the clinic consistently throughout each dayAbility to carry folders, a tablet, a notebook, and a cell phone (up to 20 pounds) for several hours per day
