Sangre de Cristo Community Care is looking for a Per Request Needed (PRN) Occupational Therapist to join our dynamic team in our Canon City office located in beautiful South Western Colorado, serving patients in Canon City and Pueblo, CO. What We Offer: • Competitive Salary • Mileage reimbursement • Good Sam Roadside Assistance • Generous Paid Time Off Policy • Growth oriented culture Responsibilities: • Evaluates and provides family-oriented services to the home health patient with neurological, musculoskeletal, orthopedic, and cardio-pulmonary impairments; fine motor, sensory motor, and developmental delays; as well as traumatic brain injuries and swallowing disorders. • Completes visit notes accurately and timely. • Provides patient and caregiver education. • Completes discharge planning and consults with other professionals, clinics or agencies when needed. Requirements Must be at least 21 years of age. A graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association. Registered with the National Registration Examination of the American Occupational Therapy Association. Two (2) years of clinical experience as an occupational therapist. Demonstrates good verbal and written communication, and organization skills. Possesses and maintains current CPR Certification. Must be able to comply with annual influenza vaccination policy.
