The EMR is Wellsky or Kinnser DEFINITION: A Licensed Practical Nurse who cares for patients who are convalescent, aged, or infirm; or when caring for acutely ill patients in a home environment does so under the direction of a Registered Nurse. REGULATORY REQUIREMENTS (IF APPLICABLE): Authorized to practice as a Licensed Practical Nurse in the State of Oklahoma. MINIMUM QUALIFICATIONS: PREFERRED QUALIFICATIONS: Two years of medical-surgical experience preferred. May consider school attendance record and performance record. Some positions may require additional skills or credentials. Have a command of the English language; have knowledge of nursing procedures, treatment and techniques, and must know basic charting procedures. Must be CPR certified on an annual basis. Should have qualifications and abilities similar to those personnel providing comparable services in the hospital. Must have a functional home telephone. PRINCIPLE RESPONSIBILITIES: 1. Participates and implements age specific care and interaction. 1.1. Employee has attended departmental orientation (training) and annual inservice education for age specific training and implements in work area what was learned in age specific training. 2. Obtains daily schedule of patients to be seen, and verifies each appointment by calling patients. 2.1. Ensures all patients scheduled will be seen. 2.2. Patients are informed if their scheduled time is unable to be kept and an estimated time of arrival given, i.e. am or pm. 3. Identifies common recurrent patient problems, symptoms, and behavioral changes. 3.1. Documentation reflects these areas in the assessment completed on the nurses' notes. 4. Involves and instructs the patient, family, and/or caregiver in the developing of the treatment plan. 4.1. Instructions are documented on the patients nursing notes and signed by the nurse. 5. Adds to the treatment plan and medication sheet when specific patient care is indicated. 6. Administers prescribed treatments as permitted by state law. 6.1. Documentation reflects physician's orders and interventions received by the Licensed Nurse. 7. Informs the physician and other personnel of changes in the patient's condition and needs. 7.1. Prompt physician contact is documented with telephone orders implemented without error. 8. Instructions to the patient, family, and/or caregiver are documented when completed. 8.1. Documentation will reflect education and instructions given to patients/families. For example, brochures, handouts and flyers. 9. Provides patient, family, and/or caregiver specific literature pertaining to their specific treatment plan. 9.1. Literature will be left in the patient's home as supplemental reference. 10. Submits daily patient visit and mileage log, supplies charges, and notes whether patients were seen on the schedule. 10.1. Nurse's notes are signed and completed within one business day in most instances. 11. Obtains and reviews patient evaluation forms, patient treatment plans, and/or auxiliary progress notes by other members of the health care team. 11.1. Plan of treatment and written nursing notes reflect inclusion of other medical information. 12. Participates in orientation of Home Health Care staff members. 12.1. Proper training in pertinent areas of the department is completed to ensure good quality care of all patients. 13. Participates fully in the departments Performance Improvement Program. 13.1. Provides input and evaluation related to any monitors completed. Outcome is addressed during meeting of patient services. 14. Participates in Safety, Infection Control, Hazardous Communication Procedures and CPR in services annually. 14.1. Will have on file current CPR card and proof of in-service participation. Will have current copy of Drivers License, Auto Insurance, and LPN License. 15. Participates in patient coordination of services meetings. 15.1. Changes in patient clinical status are updated, revised with discharge planning as part of patient care, under the direction of an R.N. 16. Implements Infection Control and Safety measures per policy and procedures. 16.1. Sharps containers are utilized when indicated, gloves are worn, hand washing, and bag technique is followed according to policy. 17. Participates in meeting requirements for state health department and JCAHO surveys. 17.1. For continued accreditation and good standing of CCMH Home Health Care program. 18. Exercises strict confidentiality regarding patients on services when outside the Home Health Department setting. 18.1. No discussion regarding patients on services occurs outside of the department. PHYSICAL REQUIREMENTS: HAND AND ARM USE: 1. Must possess sufficient dexterity to prepare medications, perform venipunctures, and to give injections on a daily basis. Must be able to operate and handle medical equipment and technologically complex devices including but not limited to keyboards, monitors, and IVACs on a daily basis. Must be able to write neatly and legibly on a daily basis. LIFTING AND CARRYING: 1. Requires continuous lifting of adult and pediatric patients. Requires lifting of monitors, oxygen tanks, backboards, boxes of supplies, and other equipment on a daily basis. Must obtain assistance when weight exceeds individual ability. PUSHING AND PULLING: 1. Requires transfers and transports of patients and/or various equipment including but not limited to beds, wheelchairs, patients, audio visual equipment, carriers, carts, and monitoring equipment. Must obtain assistance when weight exceeds individual ability. STANDING/WALKING/SITTING/CLIMBING: 1. Must be able to run in an emergency, climb stairs, bend, stoop, and carry equipment. Must have a valid drivers license, automobile insurance and a dependable vehicle. SENSE OF TOUCH: 1. Must possess sense of touch in order to palpate pulses, veins, body temperature, and for assessment of possible physical abnormalities. Ability to determine different temperatures and textures. HEARING: 1. Must be able to hear patients speak, to obtain history, and to complete physical assessment, including but not limited to auscultation with a stethoscope. Must be able to hear to obtain physicians orders verbally and per telephone, and to hear various ringing alarms while performing other job duties. Ability to hear and understand through a mask. VISION: 1. Must be able to see to assess patient condition and to perform various procedures including but not limited to preparing and giving medications, evaluating cardiac, blood pressure, and pulse oximetry readings. Must have color perception to evaluate skin color, circulation, abrasions, and body fluid color. MENTAL REQUIREMENTS: MATHEMATICS: 1. Must be able to successfully pass a medication administration test with a score of 80% or greater. Must be able to count, add, subtract, multiply and divide to record information such as patient intake and output. MEMORY: 1. Must be able to remember all activities related to the care of the patient. LANGUAGE ABILITY: READING: 1. Must have reading skills in the English language to comprehend all materials regarding all aspects of patient care within the limits of the essential performance standards identified in the appropriate job description. WRITING: 1. Must be able to write in English neatly and legibly on a daily basis with a high degree of accuracy. SPEAKING: 1. Must be able to speak clearly to verbally communicate (in English) all aspects of patient care which includes but is not limited to physicians patients, family, and other health care workers. REASONING: 1. Must be able to assimilate complex data concerning patient diagnosis and in all unit procedures. Must be able to utilize inductive and deductive reasoning in the care of the patient.
