*Adapt Integrated Health Care is looking for an Older Adult Behavioral Health Specialist to join their Behavioral Health team!**$5,000 Signing bonus and up to $1,500 relocation assistance available!**We Offer:** 100% paid premiums for full family medical insurance* Waiver cash option* Dental and Vision insurance available* Generous sick and vacation time accrual rates; accrued time rolls over year-to-year.* 5% 401k employer contribution* Life insurance policy* Employee Assistance Program* Clinical supervision toward licensure is available.The Older Adult Behavioral Health Specialist is primarily responsible for the providing interagency, multi-system, cross-sector planning for better coordination between behavioral health and primary care, providing complex case consultation for older adults and for workforce development and community health and wellness promotion in the community. Part of the job as an Older Adult Behavioral Health Specialist (QMHP) is to provide direct services including evaluation and assessment of the older adult population experiencing behavioral health.The Older Adult Behavioral Health Specialist coordinates integrated behavioral health treatment services that are based on medical necessity and that support and promote behavioral health and recovery. This position will focus on the older adult population and includes intensive coordination, discharge planning and care coordination both in in-patient and out-patient settings. The OABHS will assist clients experiencing high needs, high utilization and complex behavioral health care coordination needs in navigating appropriate community services. The OABHS will focus on engaging the client in the treatment process and assisting in accessing appropriate services within the community to meet their treatment goals in order to avoid institutionalization.*Primary Responsibilities:** Concurrent care management, effectively influencing facility treatment in regard to best practices of the older adult population and ongoing stabilization and well-being of our members.* Building and promoting community partnerships for services that will assist in the client achieving treatment goals.* Assisting the client in securing community-based services, care coordination of multi-disciplinary agencies to include behavioral health, primary care, specialty medical services, housing and peer delivered services.* Coordinate focus groups, SWOT analysis, Impact Assessments.* Develop and/or strengthen partnerships in the community that provide valuable client resources for the adult population.* Facilitate clinical meetings with a multidisciplinary team and lead additional meetings to improve strategies to successfully meet fidelity,* Develop coalition of cross-system supports and keep older adults active in the coalitions. Increase community opportunities for social and civic engagements for older adults and provide productive engagement opportunities.* Adopt and implement Mental Health First Aid for Older Adults* Plan activities that promote health and wellness. For example, conduct brown bag luncheons for potential community partners, coordinate breakfast meetings with primary care providers, develop competencies and soft-skills for the target population, partner with public health initiatives such as fall prevention and SBIRT, participate in activities such as National Depression Screening Day, etc.* Provide complex case consultation by implementing a multi-disciplinary team approach.
