Seeking on-call Social Worker (MSW) Evenings/Nights 6pm-8am(Ventura County areas)Must be comfortable with computer charting, must be ok to drive to patient home settings. $32-35/hr + Mileage Reimbursement.POSITION SUMMARY The Social Worker will be an integral part ofthe Hospice Interdisciplinary Team and will assist the team in understandingthe significant social, emotional and cultural factors related to each patient'sterminal illness. The Social Workerconducts psychosocial assessments and evolutions and provided emotionalcounseling to patients/families in their place of residence - in person andtelephonically. The Social Worker worksunder the direction of a physician and supervision of the Director of PatientCare Services and serves a as resource to our patients, families and ourcommunity. Must be able to utilizecomputers and be comfortable with electronic medical records. Inquiries aboutpart time positions are welcomed. QUALIFICATIONS:
- Master of Social Work degree from a school accredited or approved by the Council on Social Work Education
- L.C.S.W. preferred, but not required
- Clinical experience relevant to the psychosocial needs of patients and families
- Excellent verbal and written communication skills
- Spanish speaking a plus, but not required
- Valid driver license and auto insurance and reliable/well maintained transportation
