Enhances the patient experience by providing a seamless navigation process from diagnosis through the continuum of care to survivorship by offering individualized assistance to patients, families and caregivers to reduce barriers to care and facilitate timely access to care. Responsible for collaborating with physicians and members of the interdisciplinary teams to triage, coordinate and consistently manage patient care by educating the patient and serving as a point of contact for patients and families. Works with patients and families to answer questions, provide emotional support and resolve any barriers, needs or issues related to the cycle of care. Responsible for serving as the patient's liaison and advocate in interactions with the entire treatment team.
**Responsibilities**
+ Facilitates the coordinator of patient care services to assure excellence in patient care and patient flow.
+ Assists with timely access to care by coordinating/facilitating appointments.
+ Accompanies patients to consultation appointments.
+ Follows patients from diagnosis through survivorship.
+ Provides seamless referrals to resources to ensure elimination of barriers to care.
+ Assess patient's current and future needs, provides patient access to resources (internal and external), facilitates distress screenings and patient needs assessments and makes appropriate referrals as needed. Acts as a patient's advocate.
+ Facilitates the development, implementation and adherence to clinical and research guidelines, as appropriate, to ensure optimal outcomes.
+ Works with research teams and physicians to make sure patients are appropriately screened for possible participation in research trials.
+ Establishes and maintains close collaboration with physicians and the cancer care team to ensure seamless patient care and care coordination.
+ Actively participates in a Multi-disciplinary Conference.
+ Provides patient education and symptom management support.
+ Tracks navigation program metrics and quality indicators; documents patient interactions and progression and communicates navigation activities to physicians and the cancer team.
+ Documents all patient care and telephone communications.
+ Collaborates with director to plan, develop, implement, market and evaluate new or expanded programs and services.
+ Participates in community health promotion and awareness programs.
+ Outreach: Conduct breast health education classes with individuals and aggregates.
**Qualifications**
**Required Education:** Associates Degree, Nursing
**Required Experience:** Five years nursing experience
**Required Licensure and Certifications**
Registered Nurse License from the Texas Board of Nursing
**Required Minimum Knowledge, Skills, Abilities and Training**
Organizational and Planning techniques. Effective Written and Verbal Communication. Time Management. Computer skills in database management and forms development.
**Pay Range**
$30.47 - $44.19 /hour
