Your Specific Responsibilities:

This position provides Health Care as an Advanced Practice Registered Nurse Specialist (APRN) in the clinical setting at the Department of Health in Hernando County (DOH-Hernando). The incumbent in this position is responsible for providing direct disease and public health awareness intervention services to clients of DOH-Hernando including Family Planning, Sexually Transmitted Infections/Diseases (STI/STD), HIV/PrEP, TB, Refugee Health programs in accordance with a collaborative practice agreement using established DOH protocols, policies and procedures. This position requires knowledge of the public health system and the professional skills to interact with patients, health care providers and others in the community and the ability to interact with people with diverse social, economic and cultural lifestyles and characteristics. This position directly works under protocol from the Medical Director with direction from the CHD Administrator. TB testing and certain immunizations may be required. The clinic locations are in Spring Hill and Brooksville, with travel as needed between sites as a requirement for this position.

Responsibilities include, but are not limited to:

Provides direct services to Family Planning, STI, TB, and Refugee Health patients

* Assesses, evaluates, and manages patients in accordance with current APRN professional practice standards, evidence-based standards, and medical treatment guidelines and in consultation with supervising physician when indicated

* Documents all observations and actions accurately and in accordance with program guidelines including prescribed timelines

* Collects data pursuant to and evaluates patient complaints, medical histories, and physical findings

* Orders diagnostic laboratory tests for evaluation and management of patients. Interprets and evaluates laboratory results

* Provides preventive health care services in relation to programmatic guidelines

* Evaluates and initiates or modifies patient treatment plans

* Prescribes medications and accurately documents medications in the electronic medical record

* Provides referrals to physician or specialty care when indicated

* Provides information/education to patients and/or their families upon a diagnosis including a plan of care

* Provides verbal and written instructions regarding family planning methods

* Obtains informed consent

* Performs assessment, screening, and counseling

* Obtains Pap smears and provides follow-up for abnormal Pap smears

* Provides pre-conceptual and inter-conceptual health services

* Inserts and removes IUDs and implantable subdermal contraceptives

* Assists patients to make decisions regarding the use of oral contraceptives, Depo-Provera injections, natural family planning, and long-acting reversible contraceptives

* Provides pregnancy testing and counseling

* Manages and treats sexually transmitted infections (STI)

* Collaborates with the Disease Intervention Specialist to ensure STI treatment is received and documented

* Provides tubal ligation and vasectomy counseling

* Follows Family Planning Title X Administrative and Clinical Manuals and protocols guidelines

* Collaborates with the Refugee Health nurse to provide physical examinations and follow-up referrals as warranted

* Documents all observations and actions in an accurate and timely manner in the Health Management System (HMS)

* Responds to patient complaints

Plans, promotes and coordinates clinical care services

* Educates the public and promotes DOH services at various events and tracks using an outreach log

* Participates in identifying, planning and achieving short and long-term clinic and community health goals

* Contributes to the success of DOH-Charlotte programs and initiatives through participation in conferences and meetings including community partners

* Serves as an advocate for underserved populations

Utilizes a total quality management approach to provide efficient and effective services that meet organizational, customer service, internal process, and financial goals

* Assesses the health and health care needs of adults and children and makes service delivery recommendations based on quantitative and qualitative data as well as feedback from community partners and consumers

* Actively participates in performance and process improvement initiatives

* Serves as a role model, coach, and mentor for staff and students

Other Responsibilities:

* Provide training, orientation and serve as a resource to clinical support, medical and nursing staff.

* Ensure policies and procedures are in alignment with the FLDPH technical assistance guidelines.

* Reviews charts on a weekly basis for case management, as well as quality of care.

* Participate in community outreach events to promote healthy behaviors and reduce chronic disease through education and establish collaborative partnerships as indicated.

* Actively participates in efforts to improve clinic efficiency, flow and quality of care and provide input and feedback for standard operating procedures.

* Maintains and updates professional knowledge by utilizing education opportunities.

* Complete annual Department of Health (DOH), Hernando County Health Department (HCHD) and Title X training as indicated.

* Maintain a current nursing license as well as maintaining Family Nurse Practitioner Board Certification License.

* Participate in staff meetings and meetings related to patient care and clinic processes and provide staff education, if needed.

* Serves as a preceptor for Advanced Registered Nurse Practitioner (APRN) students from approved colleges, Sexual Assault Nurse Examiner (SANE) team members and IUD insertions/removals.

* Performs office procedures which are within scope of practice, insertion/removal and management of: LARCS, endometrial biopsies, vaginal/cervical polyps and colposcopies.

* Ability to collect, interpret and analyze statistical, demographic, and service data.

* Ability to formulate and monitor nursing standards and operating policies and procedures.

* Ability to develop and implement various program monitoring methodologies and techniques.

* Ability to communicate effectively.

* Ability to establish and maintain effective working relationships with others.

Qualifications:

* A master's degree from a U.S. accredited college or university with major in nursing and licensure as an Advanced Practice Registered Nurse in accordance with Florida Statute 464 or eligible to practice nursing in accordance with the Florida Administrative Code 210-8.27 is required

* Current AHA certification in cardiopulmonary resuscitation (CPR) is preferred

* Possession of a valid Florida driver's license and/or reliable transportation are required

* Experience working in STD/HIV and/or Family Planning program is preferred

* Ability to perform IUD/Nexplanon insertions/removals is preferred

* Ability to work 3x a week

EMERGENCY DUTY: Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or perform other emergency duties including, but not limited to, responses to or threats involving any disaster or threat thereof, man-made or natural.
