Airport Blvd Sanford, FL 32773The Florida Department of Health in Seminole County is looking for a motivated, hardworking individuals to fill OPS Health Educator positions located in Sanford, FL. Condition of Employment: This position may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disasters or threat of disaster, man-made or natural. This position will maintain confidentiality and security of records as specified by departmental regulations and laws.This position requires a Valid Florida Driver License. Your Specific Responsibilities:This is highly confidential and specialized position dedicated to providing services for persons living with HIV/AIDS. This dedicated care position is part of a team that are providing enhanced care coordination services to persons living with HIV in Seminole County. Main goals are to coordinate activities for the AIDS Drugs Assistance Program (ADAP), increase the numbers of persons living with HIV/AIDS who engage and remain engaged in HIV primary-specialty care, increase the number of persons who obtain insurance and increase the number of persons maintain viral load suppression. The position incumbent will be highly skilled in the performance of clerical and administrative duties and will also work independently gathering data, preparing reports, and providing day-to-day operational assistance to the Program Manager, and other staff as needed. The position will be responsible for determining client eligibility, enroll and recertify clients in AIDS Drug Assistance Program (ADAP): insurance availability & coverage; prescription(s) medication dispenses; labs; in ADAP Provide database. Tracks client eligibility and re-determination dates as well as adherence. Direct services include:
  • Registers and updates client information in the statewide ADAP
  • tracking system. b) Conducts interviews to determine client eligibility for ADAP and other Ryan White Part A/B and General Revenue-funded programs. c) Serves as a resource person for ADAP clients and assists clients in accessing and utilizing community services such as social, legal, and medical.d) Acts as liaison with case management and community agencies to ensure access, understanding various clinical/drug regimens and coordinate within the system of care.e) Promotes ADAP program in Seminole County to ensure all health care providers are aware of the program to maximize program use. f) Provide secondary prevention information as needed.ADAP worker will maintain and review client files for every new enrollment and re-enrollment to ensure files contain the required documentation for determining client eligibility. Maintains back-up files of all reports for audit purposes. Assists other ADAP workers to ensure their access to answers on ADAP issues and with daily questions or problems as they occur. Assists Program Manager and/or designee with the coordination of pharmacy services for approved clients as well as with additional areas including pilot studies, surveys, focus groups, reports, etc. Maintains 100 % compliance with confidentiality protocols as well as the security and integrity of the ADAP Electronic Enrollment System as determined by the Florida Department of Health HIV/AIDS Section of the Bureau of Communicable Diseases, as well as all other databases, including Medicaid, Health Management System (HMS), Availity, Change Revenue as assigned. Reports immediately to the ADAP Manager or designee of any problems/complaint's clients report, as the needs of the client's mandate. Works as a liaison between the patient, the ADAP program, the pharmacies and all health care professionals serving the patient, including but not limited to physicians, nurses, case managers, social workers, etc. to ensure proper access to and coordination of services; participates in all quality assurance activities and improvement issues as determined and needed by the ADAP Program and the Seminole County Specialty Care HIV/AIDS Section.Participates in Quality Control and Quality Assurance activities of the ADAP Program in Seminole, which include, coordinating the collection of ADAP Premium Assistance official clients' pickup histories from retail pharmacies not using the required CVS/Caremark copayment card, updating the Provide database, improving the pickup indicator among Premium Assistance clients and the program overall, reducing the number of 90-day suspension of services among Premium Assistance clients.Participate in ADAP, HIV treatment adherence, HIV/AIDS education/prevention and care management trainings and webinars. Provides information and training as needed in the field to community-based organizations and case managers. Remain current with changes and usage of ADAP and Ryan White policies and procedures.Incumbent will assist with chart reviews and other QA/QI activities to improve clinic functions and to meet DOH-Seminole's mission statement and goals. Facilitate the Quality Advisory Committee, maintaining appropriate relations with internal - external partners. Conduct outreach activities for potential clients to educate and promote the availability of services. Incumbent will participate in health fairs, community events and collaborate with providers among other activities. Enter all client encounters - both face to face and by phone into the Health Management System (HMS) and Provide Enterprise System for tracking and reporting purposes. Submits required reports, maintains assigned records, completes/returns all paperwork pertaining to job duties accurately, legibly and in proper order. This would also include timely submittal of timesheets, mileage, and EARS. Verifies client demographics, insurance coverage and income, accurately updating client HMS record as indicated; scans all required documents, correctly linking documents to client HMS record. Schedules appointments accurately according to appropriate site and schedule template; completes check-in of clients for appointments or on a walk-in basis in a timely manner; accurately documents 'No Shows' in EHR; answers incoming telephone calls in a timely manner; speaks clearly and concisely, exercising tact and providing accurate information; accurately and efficiently provides support in other clerical areas as needed; interacts positively and professionally with nurses and other clinic staff to assure client needs are met.Actively participate in case discussions or case conferences for the clients to improve assessment and to sustain retention in care. Participate in team meetings and all meetings convened by the ADAP Program, DOH-Seminole, Community Partners. Transcribe minutes from meetings and produces minutes for distribution within 2 days. Maintain appropriate relationships with Key Point of Entry (KPOE) into the healthcare system.This position may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Ability to prepare reports, understand and apply applicable rules, regulations, policies and procedures relating to Ryan White, ADAP, State and Local Specialty Care HIV/AIDS Programs, understand and follow legislative policy direction as established in Federal and Florida Statutes; deal with the public in a tactful and courteous manner; work independently, plan, organize and coordinate work assignments, communicate effectively, establish and maintain effective working relationships with others; utilize problem solving techniques and use personal computers.Qualifications:Minimum:Do you have at least 1 year of case management experience (interviewing, counseling), and/or 1 year of case management with at risk populations?
