Your Specific Responsibilities:Provides care coordination services including home visits to assist pregnant women, postpartum women, and infants/ children up to age three to obtain the health care and social support needed to reduce the risks for poor maternal and child health outcomes. This position will primarily serve high risk participants with health and safety concerns including but not limited to substance misuse, domestic violence, mental health concerns, and families under investigation for child abuse. These services are provided as outlined by the Florida Healthy Start Standards and Guidelines and as determined by the contracts between the Healthy Start Coalition of Pasco County and the Florida Department of Health in Pasco County.
Required Knowledge, Skills, and Abilities:Ability to advocate on the participants' behalf.Ability to communicate verbally and in writing with health care providers and other community partners. Knowledge of Community resources for at-risk pregnant and interconception women, infants, young children, and their families and procedures for accessing them.Maternal and child health principles, including; normal pregnancy anatomy, physiology, comfort measures, breastfeeding, danger signals, and what to do if danger signals occur. Normal infant and child growth and development and corresponding anticipatory guidance to promote optimal physical and emotional health, safety, and wellbeing for participant and family.Basic content of prenatal and well child- care.Importance of appropriate interpregnancy interval and basic knowledge of family planning methods. Recognition of stage of readiness to stop tobacco and other substance abuse and appropriate intervention.Family dynamics including principles related to parents adjusting to role as care giver, appropriatediscipline, parental/caregiver self-care and managing stress.Effects social determinants of health and barriers to heath equity have on birth outcomes and overall health.Child abuse or neglect indicators and the responsibility to report to the Florida Abuse Hotline.
