Job Title: Certified Occupational Therapy Assistant (COTA) Job Specialty: Rehabilitation Therapy Job Duration: 13 weeks Shift: Monday - Friday Guaranteed Hours: 32 per week Experience: New graduates are welcome; Skilled Nursing Facility (SNF) experience preferred License: State License Verification Required Certifications: Must be a Certified Occupational Therapy Assistant Must-Have: COVID test 72 hours prior to start date with results issued 48 hours prior to start date Job Description:- Ensure prescreen documentation is completed and attached when submitting a candidate.- Attach resume and license verification for submission consideration.- Provide three references for background check.- Complete county criminal background check for all counties lived and worked in over the past seven years.- Complete federal criminal background check for the past seven years or more.- Conduct a multi-state criminal search for the past seven years or more.- Perform a nationwide sex offender search for the past seven years or more.- Participate in HHS/OIG search.- Engage in GSA search.- Provide Social Security Number trace.- Submit proof of 2-Step PPD or Quantiferon test, or proof of positive PPD and Chest X-Ray.- Participate in SAM search.- Undergo a state-run background check.
