The Ortho RN Patient Navigator position is within the Case Management department of the hospital. As an Ortho RN Patient Navigator you will manage the Ortho cases utilizing the nursing process to provide optimal quality and continuity of nursing care from the pre-operative setting through a sixty-day episode of care. The Ortho RN Patient Navigator will assist the patient throughout the transition of care to ensure their well-being and achievement of the desired outcomes. Will work collaboratively as part of a multi-disciplinary team to provide patient and family education, communication with health care providers, and coordination and facilitation of services within an episode of care. Minimum 2-3 years nursing experience in a hospital setting; case management experience required. Experience with orthopedic specialty preferred. Licenses/Certifications: NYS RN licensed required.
