Hampton Behavioral Health Center is seeking dynamic and talented Assessment Coordinator/Patient Navigator for Hampton Counseling Center to assist with patients intakes in our Outpatient department. Shift:
* Full Time
* 8:00 am - 4:30 pm // Monday - Friday
Responsibilities:
Facilitates the efficient operation of the Hampton Counseling Center and Admissions Department by providing the highest level of customer service and support to referral agencies and internal Departments for patient seeking all outpatient levels of care.
Duties shall include, but not limited to:
* Responsible for completing therapeutic face to face level of care / biopsychosocial assessments on patients seeking evaluation for psychiatric treatment, with possible co-occurring substance abuse;
* Provide treatment recommendations based on outcome of the therapeutic face to face level of care / biopsychosocial assessments and initiate preliminary treatment plan;
* Provide crisis and supportive therapy with patients and families;
* Responsible for accurate and timely documentation;
* Clinically evaluating referrals for outpatient services and coordinating with referral agencies in facilitating step down/direct transfers admissions to our continuum of care;
* Providing clinical assessment information and initial treatment recommendations to insurance companies for authorizations of recommended treatment;
* Providing excellent communication skills in fielding inquiry calls and/or referral information;
* Provide brief therapeutic / crisis interventions to inquiry calls to those seeking treatment;
* Completing the clerical process for outpatient admissions in MedSeries
* Assumes clincal responsibility for all aspects of biospychosocial assessments;
* Responsible for creation and impletemation of the initial treament plan based upon biospychsocial assessment;
* Repsonsible for initialing and implementing discharge planning as needed;
* Participation as a member of mulitdisciplinary treatment team;
* Responsible for providing clincial evidence based theraputic interventions group / family therapy;
* Meet with all eligible inpatients to discuss the benefits of PHP/lOP services as part of their continuum of care treatment and ensure proper referral is made.
* Work with inpatient treatment team to assist with the facilitation of step down to HCC. In patient workflow process as it pertains to case management and discharge planning.
* Data entry, program evaluation and outcomes management via Midas
* Uses an understanding of the principles of growth and development to assess each patient's age specific needs and provide age specific treatment and care;
* Reports to the Director of Hampton Counseling Center and collaborates with the Director of Admissions and Access Center to ensure compliance with state and facility requirements
* Flexibility with covering multiple Outpatient locations;
* Must be able to maintain strict confidentiality.
Knowledge and Skills:
* Knowledge of crisis intervention techniques.
* Strong verbal and written communication skills.
* Significant computer skills, data entry, word processing, and spreadsheet Maintenance.
* Ability to multitask and strong organizational skills.

Qualifications
Requirements:
* Master's degree in counseling/psychology/social work/related field; with a current New Jersey state license as a LAC/LSW/LPC/LCSW.
* Minimum of one-year experience in psychiatric field required.
* Previous experience in crisis intervention preferred.
