Stamford Health is looking for an Outpatient Clinical Dietitian to join their Clinical Nutrition Department! The Outpatient Dietitian will function as a member of the professional health care team providing clinical nutrition care in the Tully outpatient setting. Provides nutrition education to a variety of patient populations with different disease states using evidence based nutrition standards of practice in an environment that ensures customer satisfaction levels are met.
MAJOR ACCOUNTABILITIES/CRITICAL RESPONSIBILITIES
* Serves as one of the primary nutrition resources at the Tully center.
* Educates and counsels patients, families and/or caretakers on recommended medical nutrition therapies through individual counseling, group classes and/or community presentations.
* Formulates individualized nutrition care plans based on assessment of patient needs using the Nutrition Care Process following nutrition standards of care
* Documents patient nutrition assessment, education and progress notes in the patient's medical record
* Serves as the primary point of contact for all nutrition consult referral requests from healthcare team and schedules nutrition visits on a timely basis utilizing tools to register patients.
* Conduct nutrition education classes
* Participates as an integral member of the interdisciplinary team by interacting with other members of the patient's healthcare team as needed.
* Represents Stamford Health by participating in relevant community events.
* Participates in research studies, performance improvement projects and special projects as needed.
* Stays current on the latest evidence-based nutrition care through participation in on-going professional development
* Involved with on-going development of policies and procedures and nutrition standards for outpatient nutrition education.
* Adhere to recognized standards of practice and standards of professional performance for registered dietitians as defined by the Academy of Nutrition and dietetics
Education:
Master's Degree in Nutrition/Dietetics or in Master's Program with Clinical requirements completed. Bachelor's degree with extensive experience would be considered.
Licensing/Certification/Registration:
Must have RD and be eligible for CD-N and successfully obtain within 6 months of employment
