POSITION SUMMARY:The Licensed Practical Nurse/LVN is responsible for following assigned duties. The LPN/LVN will provide appropriate nursing interventions as indicated for the care and safety of the patients. Works as a team member to facility completion of the admission and discharge process for all patients assigned. Transcribes orders accurately and audits the medical record for accuracy of the orders being transcribed and implemented. Assumes the role of patient advocate providing education to patients and families as indicated. Communicates with other members of the nursing team to fulfill the needs of the patients. Assist with activities of daily living. Encourages and fosters independence with the patients as indicated. Communicates with the medical staff on identified patient issues. Performs patient care assessments timely and accurately. Addresses patient care needs in a timely manner. Documents findings into the medical record accurately and timely. Ensures daily documentation is completed thoroughly and signed and dated according to protocol.Adheres to Human Resource policies regarding code of conduct and attendance. Follows Infection Control and Employee Health Guidelines to ensure safety of patients and staff.Actively works with the Service Development Department to facilitate admissions into the facility in a timely manner. Functions as a member of the multidisciplinary team and communicates identified patient goals, interventions, and assessments.WORKING CONDITONS:Work environment characteristics are representative of those that must be met by an employee to successfully perform the essential functions of this job. Work requires spending approximately 75% or more of the time inside a building that offers protection from weather conditions but not necessarily from temperature changes. The employee must provide documentation of a tuberculosis test during new-hire orientation and annually thereafter.
- PHYSICAL REQUIREMENTS: Must be able to stand on feet for moderate periods of time, standing couls be as high as 60-80% per working shift per day. Must be able to sit for extended periods of time, at least 4 hours. A certain amount of walking and standing are often necessary to carry out job duties. Lifting/carrying may exceed 10 pounds.
- Additional work environment characteristics may include: performing repetitive tasks/motion, and hearing alarms, telephone, and normal speaking voice. Stress is identified as a job hazard resulting from a hectic environment with frequent interruptions. The environment alone presents a certain amount of risk servicing psychiatric patients in acute and crisis needs
