Job Summary:The Nurse Navigator RN (Oncology Certified) is responsible for guiding patients through their cancer treatment journey by providing personalized care, support, and coordination. This role ensures that patients have access to the right care at the right time, collaborating with healthcare teams and acting as a liaison between the patient, family, and medical providers. The Nurse Navigator plays a critical role in offering clinical expertise, emotional support, and educational resources to oncology patients.Key Responsibilities:Serve as the primary point of contact for oncology patients, guiding them through diagnosis, treatment, and follow-up care.Coordinate care between multidisciplinary teams, including oncologists, radiologists, and other specialists.Provide patient education on cancer treatment options, side effects, and symptom management.Assist patients with scheduling, appointments, and referrals for specialized treatments.Monitor patient progress, addressing concerns and identifying potential barriers to care.Ensure timely access to services and treatments to meet the unique needs of each patient.Offer emotional support to patients and their families, helping them navigate the healthcare system and available resources.Maintain accurate and thorough patient records, ensuring communication across care teams.Participate in continuous education and training to stay updated on best practices in oncology care and patient navigation.Required Qualifications:BSN degree mandatory4-5 years of oncology nursing experience required; experience in an outpatient setting is preferred.Maryland Registered Nurse License upon hire.American Heart Association CPR Certification upon hire.Oncology Nurse Certification (OCN) required.
