Job DescriptionThis position is accountable for those pharmacy services which require pharmaceutical education and state licensure; leadership experience and extensive clinical and professional judgment to provide quality care to Med/Surg patients. This vital resource works to establish clinical practices consisting of drug reviews for appropriateness, drug monitoring, participating in trauma codes and active involvement in the management of Med/Surg patients. These duties are performed according to departmental policies and procedures, and as scheduled by the supervisory staff. RequirementsEducation:To qualify a Bachelor of Science Degree in Pharmacy or Doctor of Pharmacy Degree is required.Licensing/Certifications:New York State Pharmacist License requiredExperience:5 years experience required with primary focus in medication Therapy Management (MTM- certification required)PGY-1 Training with at least 2 years post-residency experience as a Clinical PharmacistPGY-2 Training in Internal Medicine with at least one year experience as a Clinical Pharmacist is preferred

  • Demonstrates the ability to safely handle medications
  • Exhibits professional and respectable oral communication with co-workers, patients, outside vendors, and other health care affiliates.

  • Can proficiently read, write, speak, and understand English.
  • Is open to training/giving and receiving instructions.
    • Facilitates care by providing essential clinical activities such as, disease state management, general pharmacotherapeutic monitoring, pharmacokinetics monitoring and other crucial patient care activities
    • Serves as pharmacotherapy resource to medical staff during multidisciplinary rounds and provides drug therapy recommendations
    • Offers drug information services to physicians, nurses, and allied health professionals with regards to dosage regimens, drug interactions, adverse drug reactions and use of medications in special populations
    • Researches and develops new pharmacy protocols and training procedures related to various aspects of emergency medicine
    • Provides formal accredited educational sessions, such as medical grand rounds
    • Performs direct patient care activities such as bedside patient education
    • Performs any necessary drug therapy interventions based upon hospital formulary, protocols, and target drug programs
    • Investigates and documents adverse drug reactions
    • Participates in the quality improvement and medication-use-review activities of the department.
    • Implements appropriate drug cost-containment measures to minimize drug cost while ensuring effective and efficient care
    • Provides consultation to Pharmacy and Therapeutics committee relating to pharmacotherapy issues
    • Assists with the training of pharmacy residents, staff pharmacists, students, interns, and technicians
    • Conducts research and data analysis regarding the Med/Surg patients
    • Contributes to the pharmacy and medical literature, through therapeutic, pharmacokinetic, and pharmaco-economic articles
    • Oversees the SBH Meds-to-Beds program
