SummaryProvides professional patient care and practices physical therapy by evaluating, planning, implementing, and documenting treatment. Evaluates and treats patients in accordance with the order from the physician and provides treatment through the use of a variety of therapeutic procedures and assistive devices intended to maximize functional potential in all areas of the patient's life. Develops and implements an individualized treatment plan based on patient needs and containing achievable functional goals. Revises treatment plans, as needed, throughout the treatment process. Instructs and collaborates with Physical Therapist Assistants (PTA) regarding treatment procedures to be utilized and provides the appropriate supervision of patients being treated by the PTA. Establishes and educates patients, families and other caregivers in exercises and therapeutic home programs to be continued following discharge. Completes the appropriate documentation and billing in a timely manner. All care is provided in accordance with the philosophy and essential functions of the hospital, the rehabilitation department, the Indiana State Practice Act, the American Physical Therapy Association, and The Joint Commission.EducationMust be a graduate of an accredited school of physical therapy, with a physical therapy degree as follows: Minimum of a Bachelor's degree, if graduated prior 1/1/2016; Doctorate of Physical Therapy required, if graduated after 1/1/2016; International Candidates must be a graduate of an accredited school and possess a minimum of a Bachelor's degree in Physical Therapy. Doctorate of Physical Therapy degree preferred.Licensure/CertificationMust hold or be eligible to receive a Physical Therapist license in the State where services will be provided. State licensure must be obtained within the first 90 days of employment. Must be certified in CPR or must become certified within the first 90 days of employment. Must maintain all certifications and license according to the expiration date. American Physical Therapy Association membership preferred. If assigned to drive a company vehicle, a valid driver's license is required. If assigned to drive a personal vehicle, proof of insurance is required in addition to a valid driver's license.Addendums are required for 0153 Nurse Leader, 0249 Registry RN, 0252 Registered Nurse, 0235 Advanced Registered Nurse, and 0236 Expert Registered Nurse.
