Provides transportation of families and individuals involved with Outpatient programs to medical and/or mental health appointments.1. Transports clients to appointments as needed in the community.2. Assures availability of materials and supplies and insures the provision of a clean and safe environment for clients, families, and staff.3. Documents each request for transportation on the Transportation Record Form in the client electronic health record in a timely manner.
- Will travel to the following surrounding counties: Boone, Bourbon, Campbell, Carroll, Gallatin, Grant, Harrison, Kenton, Nicholas, Owen, Pendleton and Scott.
- Typical schedule is Monday through Friday dayshift.
- Holidays, weekends, evenings, and overtime may be required.
- Traveling/driving that could at times be for extended periods of time.
This position can potentially serve clients in the following Kentucky counties: Bourbon, Harrison, Nicholas, Scott and the surrounding area.Job Type: Full-timeSalary: $16.00 per hourBenefits:
