You will be involved in launching a new Program of All-Inclusive Care for the Elderly model-centered on the belief that the well-being of seniors with chronic care needs and their families are better served in the community.Bring your expertise and skills as a member of the IDT to help manage the complex medical, functional, and social needs of older adults as we implement our Northern Kentucky PACE program. Ultimately, PACE will work to maximize the dignity and unique needs of seniors and their caregivers.Senior Community Care of Northern Kentucky is part of Volunteers of America National Services which serves as the Housing and Healthcare affiliate of the Volunteers of America parent organization.Job Highlights:403(b) Retirement Plan;Career scholarships;Quality training, continuing career education and leadership programs;Medical, Dental and Vision InsurancePaid Time Off (Vacation, Holiday & Sick Days)Position Details:Salary: $27-$32 based on experienceSchedule: M-F, occasional weekendsUnder the supervision of the Center Director, the Outreach Coordinator is responsible for obtaining initial information relating to admissions, coordinating intake process, and providing information and referrals to community agencies as appropriate. The Enrollment Specialist promotes the Senior CommUnity Care Program to potential participants and community agencies. Onboards Participants into the Senior Community Care Program.Essential Responsibilities:In coordination with local PACE leadership and VOANS PACE resources, is responsible for the development and implementation of the PACE Marketing and Community Outreach Plan and associated activities following State and Federal guidelines.In conjunction with PACE leadership, strategically coordinates PACE staff to engage in community and referral source outreach activities.Provides individual and group marketing presentations to potential referral sources.Maintains consistent, close contact with existing and potential referral sources.Maintains referral tracking system, mailing list, and marketing files.Participates in marketing activities, e.g. senior fairs, canvassing senior hi-rises, as developed in marketing plan.Plans and coordinates open houses/marketing events to enhance exposure for Senior CommUnity Care program.Attends staff meetings and in-service training as required.Follows all Senior CommUnity Care Policies and Procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.Practices Universal Precautions and follows appropriate infection control procedures.Participates in and supports the Quality Improvement Initiatives.Participates in continuing education classes and any required staff and training meetings.Gather necessary documentation and application requirements in working with participants for Medicaid approval.Ensures compliance with all State Medicaid Rules and Regulations that pertain to Medicaid, Straight Medicaid, LTC, Spousal Impoverishment and Trusts, Food Stamps, Old Age Pension (OAP) and other Medicaid programs for which participants may be eligible.Guides participants through financial eligibility process.Interpret program regulations and other pertinent material to applicants, recipients, and the general public.Receives and provides information to referrals to the Senior CommUnity Care Program; refers individuals to other appropriate resources as indicated.Assesses potential participants in home, nursing homes, hospitals or other settings as part of the intake process.Maintains current, written assessment and intake records inclusive of potential participant's expressed wishes and the referral services provided, as appropriate.QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
