As an Outreach Nurse, you'll provide nursing support and coverage for various providers across our clinics, ensuring high-quality care while assisting with outreach events. Essential duties and responsibilities:

* Check in patients for visits with the nurse/health care provider, i.e., vital signs; assess needs for that particular visit.

* Phone interaction with patients:

* First line person to evaluate patient's need to see or to speak with provider in clinic.

* Evaluate for potential emergencies and work into the schedule as necessary when schedule is full. Communicate with providers in the clinic to determine urgency of health problem.

* Follow up with patients regarding appointments, laboratory tests and other procedures.

* Arrange follow up with provider when necessary by making 3 phone calls and if condition requires, a home visit should be attempted.

* Send written notice to patient according to established protocols, i.e., pap smears, immunizations, etc.

* Records any contact with patients in chart/outreach form (Patient identifiers verified prior to any documentation).

* Makes notations on all phone contacts.

* Records any tests and procedures, i.e., blood pressure check, throat cultures, medication refills, etc.

* Documents all initial patient check in data per policy. (Ht., Wt., Assessment of pain, LMP, Meds, Allergies, Chief Complaint).

* Performs laboratory tests and treatments according to protocol.

* Prepare area for screening and assist other staff as necessary.

* Checks nursing box for adequate supplies, orders supplies as needed.

* Schedule x-rays or other diagnostic tests.

* Has equipment and supplies ready as necessary for screening event.

* Participates in Patient Education.

* Prepare packets for handouts

* Ensure that the overall operations of outreach events are performed appropriately

* Efficient in answering phone and phone calls ensuring great customer service.

* Comes prepared and on time to all events.

* Weekly inventory is completed

* Assists with set up and break down of equipment before and after events.

* Participates in in-services on a regular basis.

* Is supportive and attends in-services provided by Valley-Wide Health Systems

* Knowledgeable about emergency calls and current CPR

* Comply with all CLIA, JCAHO, and other certifying agency regulations and standards.

* Participate in quarterly proficiency testing

* Administers medication orally, intramuscularly, intravenously using excellent technique.

* Calculates drug doses accurately.

* Provides appropriate emergency care

Job Qualifications:

* Medical skills, equipment, and technology used in the nursing area

* Bilingual skills are preferred

Experience: Health care experience; lab facilities and services

Education or Formal Training: Current licensure as LPN or RN required; BLS certification/training and certification
