Job Description:

SUMMARY OF ESSENTIAL DUTIES AND RESPONSIBILITIES:

Provide outreach to patients to offer assistance scheduling appointments for needed services.

Reviewing relevant information in the Electronic Medical Record.

Scheduling appointments for patients.

Tracking the results of outreach calls made to patients

Understanding the value-based payment metrics driving patient outreach

Communicating with patients in a caring, empathic and culturally competent manner

Other duties as assigned.

Minimum Qualifications:

1. A Baccalaureate Degree from an accredited college or university in Business Administration, Community Health, Psychology or related discipline with one year of full-time experience in a supervisory or administrative capacity in business management systems, general administration, health care and medical/patient systems administration; or,

2. A satisfactory combination of education, training and experience.

Department Preferences

Some college education preferred

Basic Excel skills

Patient or customer outreach/interactions

Customer service experience is a plus

Healthcare experience is a plus

Microsoft Excel, Outlook

Microsoft PowerPoint is a plus

Experience with EPIC is a plus

Bilingual is a plus

Strong Communication skills

Organized

Experience meeting productivity targets is a plus

Cultural competency and appreciation for health equity

Experience with NYC H+H and/or the communities in which is serves.

Some college education preferred
