Accordingly, the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a CBA ) between Housing Works and the RWDSU This position requires applicants to successfully complete a background check through the New York State Office of Children and Family Services when it supports the mandate under Section 424-a of the Social Services Law for legally authorized agencies to inquire of the Statewide Central Register of Child Abuse and Maltreatment (SCR) as to the existence of any indicated reports of child abuse and maltreatment against an applicant prior to employment, certification, or licensure in the childcare field. All background checks will be conducted and reviewed in accordance with applicable federal, state and local law. Compensation: $17/hrOverview: The Outreach Specialist works collaboratively with medical front desk staff for the purpose of making the critical linkages to health care services for community members and health center clients. The Outreach Specialist serves to ensure consistent client engagement in care by conducting frequent outreach. Responsibilities:

  • Wide range of outreach and escort activities with a variety of clients with varied special needs.
  • Outreach to reengage clients for care, case management and supportive services.
  • Outreach to clients who have no showed and reminder calls to clients before their appointments.
  • Register and schedule patient appointments in ECW as needed.
  • Participate in weekly staff meetings and monthly continuing education sessions.4
  • Participate in Housing Works advocacy events (minimum 4 per year July-June) Present the health and supportive services of Housing Works Community Healthcare at outreach events such as health fairs, etc.
  • Facilitate post-hospitalization outreach to clients and schedule for continuity appointments with their respective provider(s).
  • Complete scanning of medical records into EMR.
  • Respond to patient phone calls and facilitate appointment scheduling as needed.
  • Perform other related duties as required.
Minimum Requirements
  • Strong writing, reading, listening, and speaking communications skills.
  • Able to set and observe appropriate boundaries with clients.
  • Possess well-developed decision-making skills and attention to detail with a high level of accuracy.
  • Ability to excel in a diverse, collaborative team environment. Interest in and ability to work with people of all socio-economic statuses.
  • Possess a high level of interpersonal skills to handle sensitive and confidential client information and situations.
  • Able to multi-task and remain calm in demanding and unpredictable situations.
  • Able to maintain a professional, customer service-oriented attitude always.
  • Ability to work and communicate with staff, clients and their families, community agencies, etc. to meet their needs in a polite, courteous and cooperative manner, yet appropriate assertive as situation may demand.
  • Display a high level of initiative, effort and commitment towards completing assignments efficiently.
  • Possess excellent time management skills and the ability to work with minimum supervision.
  • Ability to work occasional evenings and weekends with advance notice.
  • Ability to travel as required to perform duties.
  • Ability to speak and write Spanish fluently preferred.
  • Must be willing to participate in Advocacy events.
EDUCATION AND WORK EXPERIENCE
  • High School diploma or higher
  • One to two years of previous experience in a medical setting or related field preferred.
  • Working knowledge of community resources and/or homelessness, HIV/AIDS, mental illness, and/or substance abuse.
  • Experienced with Microsoft Office (Word, Excel), Outlook, and the Internet.
PHYSICAL REQUIREMENTS:
  • While performing duties of this job, the employee may be regularly required to travel to patient settings including the home and to work in the health center environment.
  • Employee is also required to accept incoming calls and must be able to hear and speak.
