Job Description:

We are looking for Outreach Workers with caring hearts and a high attention to detail and data quality.

Reporting to the LCSW or LSW for Victims of Violent Crimes in each Central, South, North, East Wards in Newark.

The Outreach Worker will be responsible for engaging and maintaining contact with recent victims of violent crimes within Newark. The ideal Outreach Worker candidate will have previous experience and/or knowledge with violence intervention and prevention work. The Outreach Worker will work in tandem with the LCSW or LSW to establish trusting relationships with victims of violent crimes with limited to no access to resources. The Outreach Worker will successfully engage the individuals into services and help them to maintain the care plan ultimately to establishing better overall mental health and independence. This position requires a proactive individual with excellent interpersonal, communication and organizational skills. Most importantly, City of Newark Violence Reduction Program is looking for a person with a heart to serve.

We are recruiting for shifts for all 7 days of the week.

RESPONSIBILITIES of the Outreach Worker, Violence Reduction Program:

To engage daily with individuals, families, friends having experienced violent crime and build a trusting and supportive relationship, providing emotional support and focusing on target goals.

To encourage clients to access services for which they qualifying order to achieve goals on their service plans

Orient clients as to the services and requirements for services available

Support clinicians in initial intakes and assessment to determine client's emergency and immediate needs

Develop and maintain links to social service agencies

Maintain contact with referral sources

Act as a liaison between clients and agencies rendering services to clients within your caseload

Participate in field visits and engage clients to participate in medical/housing/mental health/substance abuse treatment appointments

Willing to engage client from where they are, i.e. in the community, in hospitals, at police stations, at crime scene, at wakes and funerals

Engaging clients in a trusting and supportive treatment partnership, providing emotional support while maintaining professional boundaries

Participate in case collaborative meetings with Project Staff

Responsible for inputting accurate data in customized database

Be able to maintain strict confidentiality

Have empathy and respect for all consumers and their families/caretakers, as well as maintenance of good rapport with outside agencies

Possess excellent time management, communication, problem solving, and organizational skills

Be able to schedule and maintain appointments, remain flexible with changing timetables, and follow up appropriately with providers

Be dependable and have the ability to maintain own schedule, while finishing paperwork
