Job Summary:

Position Title: Outreach Specialist

Job Summary: The Outreach Specialist will provide outstanding customer service to potential Health Home Members. The incumbent acts as an intermediary between providers and members - serving in numerous roles such as educator, evaluator, and service coordinator and connecting the members to community resources as needed.

Responsibilities:
  • Achieve daily enrollment targets and provide care support services to members with chronic health conditions.
  • Perform outreach activities, especially during house calls, PCP, and Specialty services to potential enrolled members. Participates in community events to publicize health home services.
  • Educate the potential members about Essen Health and the Health Home division, and answer any questions related to the health home and outreach material as needed.
  • Adhere to lead health home's outreach and enrollment standards; complete all documents associated with the LHH standards, including the DOH consent form, patient bill of rights, code of conduct, internal HH screening form, and verification of diagnosis, including the PSYCKES.
  • Using the HH internal screening, write the attestation associated with the patient's needs secondary to their qualifying chronic conditions.
  • Connect the eligible members to immediate community resources, care, and support.
  • As needed, provide immediate intervention to patients with urgent needs, such as housing and hospitalization.
  • Assist the medical office as needed, and meet with walk-ins and scheduled patients to talk about Essen Health and the health home division.
  • Call, schedule, and confirm patient appointments.
  • Enroll potential members in the Essen Health Home program and other specialty services.
  • Navigate the potential enrolled members to immediate support in the community.
  • Conduct new patient assessment screenings consistent with the scope of services.
  • Keep patients informed of progress on health home enrollment.
  • Meet with patients in their homes, community sites, clinic offices, and Health Home offices.
  • Perform field visits to enroll members into the program.
  • Complete timely the consenting documents for all new enrollments.
  • Provide enrollment productivity reports to the outreach supervisor at the end of each day.
  • Perform other duties as assigned by the Outreach Supervisor.
Qualifications:
  • A minimum of two years experience in performing community or targeted outreach or two years in healthcare, community health, supportive housing, or other social service case management.
  • Excellent customer service, organizational, interpersonal, and communication skills
  • Ability to multi-task and work under multiple priorities and deadlines in a fast-paced environment
  • Proficiency in Office Suite
  • Proficient in Spanish and other languages preferred

$16.00-$20.00 an hour
