Position Type: Non-Instructional/Outreach WorkerDate Posted: 4/27/2023Location: Early Childhood Program - Head StartOUTREACH WORKER QUALIFICATIONS: High school or General :Equivalency Diploma and three years of experience working with families in a social services or education setting. Family Service /Worker Credential preferred. (Bilingual a plus) JOB GOAL: To assist families in becoming self-sufficient by providing them the support they need to recognize and develop their own strength, establish goals and advocate for themselves. REPORTS TO:Head Start Project Site Director SUPERVISES: NIA EVALUATION: Performance of this job will be evaluated in accordance with provisions of the Board of Education's policy on evaluation of personnel. TERMS OF EMPLOYMENT: All recommendations for employment must be approved by the Head Start Policy and New Haven Board of Education. $23,397 - $30,343 per 3429 Paraprofessional Contract PERFORMANCE RESPONSIBILITIES: Provides direct services to a maximum caseload of forty to sixty families Facilitates the development of a Family Partnership Agreement with each family on caseload. Establishes supportive relationship with families through the Family Partnership Agreement Advocates for and works with families to assist them in achieving their goals Recruits and enrolls children and families for the Head Start program Works with Social Workers to plan and implement case management plans for familieswho present with special problems. Collaborates with community agencies and Head Start staff to ensure the social services, health and mental health needs of the families are met. Provides referrals and assists families in accessing basic services. Ensures for funded enrollment and that a11 85% Average Daily Attendance (ADA) is maintained for assigned caseload. Maintains weekly and monthly data on enrollment, ADA, referrals. Participates in weekly Family Review meetings with classroom teacher.In addition, provides Lead Social Worker with weekly Family Reviews for Area Team Meetings. Conducts a minimum of two home visits a year or more as needed. Assists other service delivery areas with follow-up assistance as needed. Provides mid-day and late-day duty coverage. Attends weekly Social Service team meetings. Performs related duties as required.
