Job Description:

Under the direction and supervision of the Home Care Supervisor, the PACE Caregiver provides personal care, light housekeeping, and/or supervision services as well as other activities and tasks as assigned to assist different areas in the PACE Center, including rehab therapy, kitchen, transportation, center shower, and center laundry to North East Medical Services PACE participants in the PACE Center, and/or in the participant's home.

ESSENTIAL JOB FUNCTIONS:

* Provides assistance in the participant's home according to the participant's individualized care plan, and in coordination with the PACE Interdisciplinary Team (IDT), the participant, and the

participant's family. This may include assistance with:

* Personal care including tasks such as peri-care, grooming, dressing, showering, bathing, toileting, feeding, and ambulation.

* Housekeeping including tasks such as changing linens, laundry, and light cleaning.

* Provides assistance to participants based on primary area of assignment(s) within the PACE Center:

* Day Room: Supports participant engagement in the activity program under the direction of the Recreation Therapist while meeting their personal care needs.

* Rehab Therapy: Implements interventions, as appropriate to training, as directed by rehabilitation staff, including use of mechanical assistance for participant transfers and mobility.

* Kitchen: Assists with food service preparation and delivery and monitoring during mealtimes.

* Transportation: Accompanies drivers to pick up and drop off participants. Accompanies participants to outside clinical appointments when necessary.

* Laundry: Washes and dries Center and participant laundry. Coordinates getting participants' laundry to/from the participant's home.

* Shower: Assists participant shower at center shower room in accordance with participant's individual care plan.

* Provides supervision and emotional support as needed for participants to perform necessary tasks.

* Reports observed changes in participants' conditions to the Home Care Supervisor and/or other center-based clinical staff.

* Observes, tracks and reports participant status, including information such as vital signs, nutritional intake, and output.

* Proactively communicates participant health and safety concerns to Home Care Supervisor and/or other program staff.

* Demonstrates safe practices utilizing proper body mechanics and complying with universal precautions.

* Interacts in friendly, courteous, and professional manner with participants and their families/caregivers.

* Participates in social and recreation activities with participants as appropriate.

* Assists with covering assignments in other areas as needed for daily operations.

* Utilize Epic appropriately and efficiently and serve as content and workflow expert for resolving issues and proposing solutions in Epic.

* Performs other job duties as required by manager/supervisor.

QUALIFICATIONS:

Required:

* Minimum 1 year of experience as a Personal Care Aide, Certified Nursing Assistant, or EMT.

* Must complete and pass the North East Medical Services PACE training curriculum upon hire.

* Current BLS and First Aid required

* Ability to communicate effectively.

Preferred:

* Knowledge of the geriatric population.

* Knowledge of safe and sanitary personal care techniques for elderly population with physical with physical limitations.

* Knowledge of proper body mechanics during provisions of assistance with Activities of Daily Living.

* Home Health Aide certification

* Certified Nursing Assistant (CNA) certification

* 1-year experience working with the frail or elderly population.

LANGUAGE:

* Must be able to fluently speak, read and write English.

* Fluency in Chinese (Cantonese and/or Mandarin) is required.

* Fluency in other languages is an asset.

STATUS:

This is an FLSA Non-exempt position.

This is not an OSHA high-risk position.

Salary: $30.00/Hour
