Job Description:
Position Summary: Under the supervision of the PACE Home Care Manager, the Home Health Aide provides support to the Home Care team in the coordination and delivery of home care services to PACE participants. Home Services include- DME (durable medical equipment), delivery of meals, caregiver services, lifeline, ramps, medical supplies, oxygen tanks, etc. The position will assist with home delivery of meals and medical supplies. The position may also assist in other areas such as primary care and therapy. Essential Functions of the Job: • May provide or coordinate direct personal care to participants as directed or assigned by the Home Care Nurse. • The Home Care Aide duties if providing direct care at the home may include showering, toileting, laundry and feeding assistance and companionship services to and from medical appointments. May include ambulation and transferring assistance. • Assists other staff in carrying out program assignments. • Provides support to PACE nurses in the coordination and delivery of meals, DME, caregiver services, etc. • Will work with other disciplines as needed/assigned. • Assists in maintaining patient areas in a clean and orderly manner. Cleans and organizes program areas including. • Assists with home delivery of meals and medical supplies. • Follows infectious disease precautions to prevent the spread of organism. • Attends center staff meetings and in-service educational training programs. • Assists Home Care Team as requested, • Conducts home visit safety check list. • Assists participants with surveys. • Delivers supplies, equipment, grocery bags, and any other items as requested/needed. • Generates reports of services, may have Additional Duties and Responsibilities. Additional Duties and Responsibilities: • Communicates effectively with participants and co-workers. Works effectively • Participates in interdisciplinary team meetings as needed. • Adheres to SYHC and PACE attendance and punctuality policies and practices. • Follows established PACE and ADHC departmental and position specific policies and procedures and updates as needed. • Attends meetings as required and participates in committees as directed. • Maintains health and safety standards at all times and takes an active role in the Safety and Injury prevention in the Workplace Program; maintains all records and documentation as required. • Performs other related duties as assigned or requested. Minimum Qualifications: Must possess the skills necessary to provide care for geriatric patients and must be able to function well in a team environment. Only act within the scope of his or her authority to practice. Meet a standardized set of competencies for the specific position description established by San Diego PACE and approved by CMS before working independently. Job Requirements: Education Required (Minimum level of education): • High School Diploma, GED • Possess a Certified Nurse Assistant or the Certification of Home Health Aid • Must have a current Basic Life support card (First Aid and CPR) by the American Heart Association. Experience Required (Minimum level of experience): • One year experience with the frail or elderly population • Valid California Driver's License • Prefer bilingual (English/Spanish). Technical Knowledge and Skills Required to Perform the Job: • Basic typing and computer skills, Microsoft, Word, Outlook, Excel Working Conditions and Physical Requirements: Physical clearance for reaching, bending, stooping, crouching, kneeling, and grasping.
