Pain Medicine Management APP Opportunity
Outpatient
The purpose of the Advanced Provider is to assist the physicians of the practice with medical management, procedures and other direct patient care including diagnostic, therapeutic, and preventative care relating to chronic pain. Physician Assistants and Nurse Practitioners will work under the direct supervision of the practice doctor.
Typical work week hours are: (3 days) 8am-5pm
Location: Pain Clinic at East Campus & CMC
There are no Call responsibilities or weekend duties
Essential Job Functions:
- Perform patient history and physical examinations
- Diagnose, treat, and manage and chronic Pain related health problems
- Order, and interpret appropriate Radiologic Imaging/laboratory test when needed
- Prescribe pain related medications, including controlled substances as delegated in collaborative practice agreement
- Provide education to patients and families regarding safety of medications being prescribed
- Document assessment, management, and education clearly, concisely, within medical record
- Demonstrate strong communication skills with patients and significant others to provide feedback regarding findings and treatment plans
- Consult other health care team members in collaboration with physician to meet physician needs, while ensuring continuity of care
- Provide phone triage to assess patients needs and conditions and respond appropriately
- Coordinate patient care activities with other members of the office, hospital, or community
- Refer to appropriate specialist when needed
- Demonstrate compliance with documentation and coding requirements
- Maintain and update skills and knowledge
- Work with all members of the team to achieve patient centered care.
- Contribute to scheduled team meetings to discuss/clarify roles, responsibility, processes, and patient issues
- Provide care management by communicating with community agencies as appropriate.
