Description
Parent Advocate, Trinity
As a Family Peer Advocate, you will provide an array of services to families affiliated with Trinity Alliance who are raising children experiencing social, emotional, developmental, substance abuse and/or behavioral health. You will report to the Program Manager, Community Solutions and Peer Support. Service provision will include in home visits with families and onsite work with Service Navigators. Community-based travel is required.
Availability:Part-time position with a Monday through Friday schedule
Responsibilities:
+ Provide support, advocacy and resources to the family based on the child's needs as identified by the parent/caregiver.
+ Provide services within a Family-driven Care framework, including respecting family choice and voice, believing in each family's potential to learn and grow, and utilizing strategies that empower families.
+ Connect families to supports in their community.
+ Work with family members to identify goals and objectives of treatment.
+ Educate parents/caregivers on how to advocate if services are not meeting their needs.
+ Participate in relevant treatment meetings and attend all required trainings.
+ Complete all documentation.
Requirements and Education:
+ A High School Diploma.
+ Must be a parent/caregiver of a child with Mental Health and comfortable sharing your experience.
+ Must obtain Family Peer Advocate Credentialing through Families Together of NY.
+ Must have a valid NYS drivers license with reliable transportation.
