Qualifications:
  • Bachelor's degree + 1 year of community-based experience working with high-risk populations required.
  • Ability to conduct interviews covering extremely sensitive and confidential information
  • Strong organizational and time management skills
  • Experience working with individuals with substance abuse issues is preferred.
  • Must have a valid driver's license

Purpose: The Parent-Child Assistance Program (PCAP) is a home visitation case management intervention for high-risk mothers who have used alcohol and/or drugs during pregnancy. As a case manager with the Parent-Child Assistance Program, your purpose is to provide support and guidance to families facing challenges, ensuring that parents and children have the resources and skills they need to thrive. You are dedicated to fostering a nurturing environment where families feel heard, valued, and empowered. By offering tailored assistance, practical solutions, and compassionate advocacy, PCAP case managers strive to help families overcome barriers, strengthen their relationships, and build a stable, nurturing home. Case managers' goal is to support parents in their journey toward self-sufficiency and to create a foundation of hope and opportunity for their children, helping them toward a future marked by resilience, growth, and success.Primary responsibilities include, but are not limited to:
  • Establishing and maintaining a required caseload of 15-20 families.
  • Conducting home and community visits with clients (approximately twice a month).
  • Assisting clients in setting goals based on their individual strengths/needs assessment and identifying steps to achieve those goals while monitoring progress for successful outcomes.
  • Identifying and maintaining contact with community providers and agencies whose services are relevant to client needs and assisting with navigation of those systems.
  • Providing data input both monthly and bi-annually of client's progress.
  • Attending and participating in bi-weekly individual clinical supervisions and weekly team staff meetings.
  • Maintaining current CPR, Infant CPR, and First Aid Certification
  • Working effectively and efficiently in the field with little supervision.
