As a Care Manager, you will work within our Clinical High Risk Psychosis (CHRP) program located in Cherry Hill, Camden County NJ. As the Care Manager you will work with youth in an early identification and intervention program who are at increased risk for the development of a psychotic disorder (based on family history and/or emergence of troubling symptoms). Clinical High Risk (CHRP) provides office-based and in-community care for adolescents and young adults ages 10-25 who are assessed to be at-risk for psychosis. The goal of the program is to decrease symptoms, enable individuals to engage in social, academic and/or vocational activities, and delay the onset of psychosis. Services include psychiatric prescribing, case management, individual/family/group therapy and linkages to community resources. Schedule: Part-time; 20 hours; Two (2) days 9a-2:30p; Two (2) evenings 2:30p-8:00p; 30 min unpaid lunch Responsibilities:
- Assist Program Consumers and their families in linking with community resources as needed
- Coordinating care for the consumers by collaborating with treatment providers, school, staff and other relevant individuals
- Document all interactions with consumers, their families and providers and communicate with the team, maintain up-to-date information the consumer's electronic health record.
- Meet with consumers in their homes and community as needed in order to assist with educational, vocational and other needs
- Participate in weekly Supervision with the Team Lead
- Participate in Team Meetings and Collaborating with treatment team
- Promotes family-driven, youth-guided services and recovery-oriented environment; assists individuals and their families in making decisions, taking action and goal planning. Provides education and information to the individual and their family
- Performs other related duties as necessary

Qualifications:
- Bachelor's Degree in Social Work or related field;
- Valid driver's license in good standing.
