OverviewThe Diabetes Educator is responsible for the nutritional care and education of patients referred for diabetic management.Responsibilities
- Provides nutritional care to patients
- Assesses patient nutritional needs. Completes referral data collection, interview, history form, prioritizes needs
- Interprets, negotiates, calculates, and executes MD orders for modified diets.
- Develops and implements nutrition care plans.
- Provides written and oral education to patients and families as ordered by an MD for insulin initiation, meter instruction and oral agent instruction.
- Records and documents appropriate information in medical records. Processes and records patient charges for services.
- Evaluates patient skills and cognitive understanding by observation, discussion, study guides and follow-up calls as indicated.
- Develops, reviews and revises patient instructional material.
- Provides up to date information to the community.
- Responds to phone calls from the community.
- Communicates with other departments and disciplines within the practice.
- Provides for own professional growth and development.
- Attends CME programs on diabetes, prenatal nutrition, or other related topics.
- Maintains diabetes acumen by regular review of publications, research studies, and literature searches.
- Participates in local diabetes education meetings.
- Updates statistical data on diabetic patients monthly or as requested.Qualifications
