Part-time 3 six hour daysMaximise your chances of a successful application to this job by ensuring your CV and skills are a good match.Summary of Duties:Within the administrative structure of the Agency and the ethical and legal cannons of the profession of physical therapy, the Physical Therapist shall develop and deliver a professional program of physical therapy to developmentally disabled adults in the clinic.A Physical Therapist will be under the administrative supervision of the Article 16 Clinic Director of Treatment Coordination. New York State law mandates that a physical therapist secure a physician's authorization for most, if not all, the services provided by the profession of physical therapy. It is the responsibility of the Physical Therapist to ensure that this prior authorization is secured before any evaluative or therapeutic efforts are initiated. It is incumbent upon a physical therapist to work cooperatively with all aspects of the agency.Reporting Relationships:Receives administrative supervision from the Director of Treatment Coordination and clinical supervision from the Medical Director and Physical Therapist SupervisorResponsibilities and Illustrative Duties:1. The Physical Therapist provides a physical habilitation/rehabilitation services to developmentally disabled adults within sasi's Article 16 Clinic. Upon receipt of a referral and medical authorization. The Physical Therapist provides a professional physical therapy screening or evaluation in a timely fashion. The evaluation assesses all appropriate areas of the participant's physical functioning and identifies areas of need which may require intervention. Subsequently, the Physical Therapist prepares a written physical therapy evaluation report and a treatment plan which will make meaningful statements regarding the participant's physical capabilities, noted areas of deficiency or strength, and prognosis. The treatment plan must indicate the nature, frequency and modalities of recommended therapeutic intervention.2. The Physical Therapist provides a therapeutic program utilizing therapeutic interventions for the purpose of developing and improving, restoring or maintaining specific physical capacities of the participant.3. Provides reports required by the clinic regulations, including assessments, reviews, and treatment plans.4. Evaluates individuals for their need for assistive equipment and completes necessary paperwork for this equipment to be obtained.5. Familiarizes him or herself with the policies and procedures of the clinic and the agency.6. Initiates or attends participant related team meetings for the benefit of developing, altering, or terminating therapeutic programs.7. Maintains a working knowledge of the developments in the field of Physical Therapy by attending conferences and in-service programs and maintaining appropriate memberships in professional organizations.8.
