Job DetailsJob Location1400 Avenue Z Health Home - Brooklyn, NYPosition TypeFull TimeEducation LevelLMSW, LMHC, or LCSWSalary Range$80,000.00 - $80,000.00 SalaryJob CategoryNonprofit - Social ServicesDescriptionJOB SCOPE:Licensed Mental Health Professional to supervise a NY State Office of Mental Health (OMH) funded Adult Home Care Transition team that exclusively serves individuals transitioning from an Adult Home to the community. The team's work will involve addressing the preparatory needs of participants in the early stages of discharge planning, development of daily living skills, and coping mechanisms through group and individual work at the Adult Home. The team will follow individuals into the community using the Pathway Home model of care to ensure that they become both linked and engaged with identified community providers and that their support network is sufficient to meet their needs. The role will require some on call coverage.ESSENTIAL FUNCTIONS:Management and oversight of the day to day activities of the Pathway Home Adult Home team;Counsel in individual or group settings with emphasis on prevention and preparing for independent community living and to promote optimum mental and emotional health. May help individuals deal with issues associated with addictions and substance use; family and social relationships; stress and symptom management; activities of daily living; medication management; and housing readiness;Participate in case conferences to help identify community health needs and tailor services to meet these needs;Ensures that programmatic systems are in place and functioning to meet member needs;Ensures that internal and external regulatory agency standards and requirements are met;Provide regular individual and group supervisory support to a multi-disciplinary team and orienting new team members to policy and procedures as well as ongoing training and mentorship;Assists with the identification, development, implementation, and completion of quality improvement projects and plans to solve problems/issues;Facilitate and monitor the organizing of team member's schedule; as well as leading weekly staff meetings;Perform chart reviews and ensure completion of appropriate and timely documentation;Compile and review record requests from the Independent Reviewer, SDOH and Plaintiff's Office for selected AH class members.Collect, analyze and report data, as required, and specifically regarding metrics outlined in the proposal, including but not limited to:Frequency of enrollee contactFrequency of enrollee hospitalizations and ER visitsBehavioral Health and Physical Health AppointmentsStaff productivitySuccessful Transition to Supported Housing ServicesDevelop relationships with providers to facilitate successful transition for individuals and to expedite and solidity placement in community housing;Mange teams on-call duties;Manage recruitment of staff, as well as addressing staff performance-related issues in accordance with salient policies and procedures, specific to the organization of hire;Screen candidates for program and determine eligibility. Assign caseloads to team members;Work with all stakeholders (members, network, families, and government staff) in a caring and respectful manner, and with due understanding of and consideration for cultural differences.QualificationsSKILLS AND KNOWLEDGE:Knowledge of mental illness and serious emotional disturbances and substance abuse disorders;Knowledge of treatment, rehabilitation, and community support programs as they relate to consumers/residents, families, and staff;Knowledge of techniques for identifying and preventing potentially violent behavior, including crisis management techniques;Ability to develop, evaluate, implement and modify a clinical treatment intervention to meet the needs of individual recipients;Ability to prepare accurate and timely reports;Ability to manage multiple projects and ask for help when needed;Knowledge of Microsoft office applications, including Excel;Ability to communicate effectively with stakeholders;Competency in written, interpersonal, verbal and computational skills to present and document records in accordance with program standards;Preferred familiarity with systems and tools commonly utilized across HH programs (i.e MAPP, UAS, EMEDNY, PSYCKES).EDUCATION AND EXPERIENCE:Master's degree or higher in Social Work, Mental Health Counseling, Public Administration, Nursing, Public Health, Public Policy or a related field. A minimum of 3 to 5 years post-masters work experience in the mental health and criminal justice field with progressively increasing responsibilities, preferably with the target population.
