Job Summary:This position manages and supports individuals living with disabilities through a structured, vocational development program. This position is responsible for guiding participants in overcoming employment barriers, developing individualized plans, and connecting them with essential vocational training and resources. The Pathway Navigator plays a key role in helping participants build confidence, develop skills, and achieve sustainable, competitive employment and independent living skills.
Qualifications: - Bachelor's degree in social work, psychology, special education, vocational rehabilitation, or a related field (preferred).
- Experience collaborating with individuals living with disabilities, particularly in vocational or employment support roles.
- Knowledge of vocational rehabilitation programs, disability services, and employment-related laws and accommodations.
- Strong interpersonal and communication skills, with the ability to work effectively with a diverse range of stakeholders, including families, employers, and community partners.
- Excellent knowledge of performance evaluation techniques and key metrics.
- Proficiency in using Microsoft Office Suite programs.
- Adaptable and independent thinker with a solution-oriented mindset.
- Ethical judgment and discretion with confidential information.
- Excellent planning and organizational skills, including the ability to manage multiple projects at once, meet deadlines, and produce consistent, measurable results.
- Must have comfort and aptitude to collaborate with people of various backgrounds.
- Must have dependable transportation.
- Must have a driving record acceptable to WE Inc.'s insurance company for coverage, plus auto liability coverage that meets WE Inc.'s requirements.
Key Responsibilities: - Program Coordination: Oversee the day-to-day operations of the program, ensuring services are tailored to meet the specific needs of individuals living with physical, intellectual, and developmental disabilities.
- Individualized Support: Conduct comprehensive assessments to understand each participant's unique abilities, challenges, and vocational goals. Develop and implement personalized action plans that focus on enhancing skills and supporting professional growth.
- Resource Navigation: Connect participants with vocational training, job coaching, rehabilitation services, and community resources to remove employment barriers and foster independence.
- Career Development: Assist participants in exploring career paths, developing job readiness skills, and securing employment. Provide guidance on workplace accommodations and job modifications as needed to support long-term success.
- Collaboration: Collaborate with internal staff, external service providers, and employers to create an inclusive support network. Maintain open communication with participants' families and caregivers to ensure holistic care and assistance.
- Monitoring and Reporting: Track participant progress through regular evaluations and maintain detailed records of milestones and outcomes. Provide reports to the Director of Programs and adjust individualized plans as needed to ensure participants stay on track to meet their vocational goals.
- Training and Workshops: Facilitate group workshops and individualized training sessions focused on employment skills, personal development, and building workplace confidence for participants.
- Advocacy: Advocate on behalf of participants to ensure they receive the services and accommodations to which they are entitled. Support participants in overcoming challenges related to their disabilities in both work and personal settings.
