Shift Details: On Call/Pool, 12 Hour Shift 8 AM to 8 PM, Every Saturday and Sunday at either Jackson Memorial or a nearby Urgent Care Center in the downtown area

Position Summary: Patient Advocate will enhance the patient experience and promote greater continuity of care by assisting patients in scheduling follow-up care in advance of discharge, educating patients about the importance of follow-up care, and participating in other care continuity initiatives within the department as needed.
Duties & Responsibilities:- Directly assists patients with scheduling appropriate follow-up visits prior to their discharge from the emergency department.
- Educates patients on the importance of seeking follow-up care and actively engaging in their own care journey.
- Verify patient's insurance eligibility as needed.
- Records basic data regarding each patient interaction, such as whether the patient was able to schedule an appropriate follow-up visit.
- This position is customer service-oriented with patient-centric attitude, effective planning and organizational skills, an ability to be accountable for quality customer service, and respond appropriately to a variety of personalities and situations.
- The ability to maintain patient confidentiality is essential.

- Performs all other related job duties as assigned.
Experience: Generally requires 2 to 3 years of related experience.
Education: High school diploma or GED equivalent is required. Bachelor's degree is preferred.
