DescriptionWestern Tidewater Community Service Board welcomes you to join our team! We are seeking a Full-Time Patient Advocate position.General Responsibilities for Patient Advocate:
  • Develop a standing 'onsite' review schedule for TWC, CTH and CCSU
  • Develop a standing 'onsite' review schedule for Waiver Residential and ICF Residential
  • QA would conduct an onsite risk assessment, talk with individuals/review treatment plans and notate recommendations/issues/concerns as applicable.
  • Develop a treatment team (specific to service provider) with scheduled meeting dates/times to review case issues/risk assessments notated during onsite reviews.
  • QA would secure a copy of each programs policy manuals - this will ensure a clear understanding of staff expectations, documentation standards and accountability measures.
  • Site checklists to be available to QA during each onsite visit (fire drill schedules, staff assignment sheets, facility inspection sheets).
  • QA would complete 'unannounced' reviews of each shift (AM, PM and Overnight) camera footage for all locations at a minimum of once per month to ensure there are no intentional violations re: to human rights and quality care. Findings reported to Amy and Brandon; investigation initiated as deemed applicable.
  • Critical Case Reviews - a designated team shall be identified to review critical issues within 24 hours of the site/risk assessment. The team should include a nurse.
  • Site checklists to be available to QA during each onsite visit (fire drill schedules, staff assignment sheets, facility inspection sheets).
Qualifications and Skills for Patient Advocate:
  • BA/BS in Human Services (Preferred).
  • One-year direct care experience with individuals with SMI, SA, or who are dually diagnosed (Preferred).
  • Must be strong in conflict resolution with experience in collaboration and problem-solving with others.
  • Must possess a valid Driver's License with an acceptable DMV record
