  • Routinely round on the inpatient population to provide information on the hospitalist program.
  • Assist patients with scheduling necessary follow up care after discharged from the hospital.
  • Communicate crucial medical information to outpatient providers and ensure necessary patient follow-up has been identified.
  • Act as a resource to nursing leadership, unit managers, and ancillary staff for issues related to the hospitalist program.
  • Review electronic medical records and convey requested information to IHP providers.
  • Generate, maintain, and edit patient assignments in electronic medical record.
  • Perform office duties including answering phones, faxing, emailing and data entry as needed.
Education/Work Experience:
  • High school diploma required; associates degree preferred.
  • Administrative assistant/office management experience required
  • Three years of customer service experience; hospital experience preferred.
Required Skills:
  • Must be clean, neat, and maintain professional appearance.
  • Must have excellent interpersonal skills, customer service attributes and ability communicate effectively with both internal customers and patients. Must maintain professional and friendly demeanor at all times.
  • Highly trustworthy and able use discretion when working with confidential information.
  • Computer proficiency including email and database management skills.
  • Must enjoy working with people.

As a We Care Team member, s/he will be able to work with minimal supervision and perform a variety of care coordination tasks to promote a successful journey to health for our patients after they leave the hospital. The We Care team recognizes that healing includes more than just medical treatment and will assist our patients heal from the emotional toll a hospital stay can produce.
