**JOB SUMMARY:** The Patient Care Attendant goes to the hospital room and performs tasks that enable the client to feel personal comfort and safety as a key member of the health care team. Responsible for observing the client and reporting observations to the licensed healthcare team.

There is no hands on patient care involved. You will be monitoring for any behavioral issues, risk of falls, disorientation, etc.. You mayassist with linen changes or setting up food trays.

**Position Shift:**

* 12 hour - 7am to 7pm or 7pm to 7 am

* Full Time (36 hr week)

* Part Time (24 hr week)

The Home Health Quality team is a small task oriented team which consists of a quality educator, a quality data analyst, and quality reviewer. They work closely together with clinical managers to provide meaningful data on home health patient population and clinician best practices.

**KEY RESPONSIBILITIES:**

* Remains close to the patient at all time, and calls for help immediately if any risk to the patient is identified.

* Recognizes acute changes in patient safety conditions.

* Contacts a member of the healthcare team to report any change in condition or observations made.

* Supports patient care under the direction and direct observation of the healthcare team (such as assisting with linen changes, meal tray preparation, and filling of water pitchers).

**Basic Qualifications**

**Position Qualifications** **Required:**

* High School Diploma or GED

* **1 year of relevant experience**

**Strongly Preferred by Department:**

* Patient Care experience

* Excellent interpersonal skills

* Dependable/reliable

* Valid drivers TN license and transportation

* **Physical Requirements/Strengths needed & Physical Demands:**

+ Medium Work category requiring exertion up to 50lbs of force occasionally and/or up to 20 of force frequently and/or up to 10 of force continually to move objects.

+ Occasional: Lifting under 35 lbs: Raising and lowering objects under 35 lbs from one level to another

+ Occasional: Lifting over 35 lbs: Raising and lowering objects from one level to another, includes upward pulling over 35 lbs, with help of coworkers or assistive device

+ Occasional: Bending/Stooping: Trunk bending downward and forward by bending spine at waist requiring full use of lower extremities and back muscles
