The Patient Care Coordinator is responsible for scheduling all new patient evaluations, follow up appointments, answering phones and mapping routes for home visits. Location: 7430 Remcon Circle, El Paso, TX (Building B) Schedule: Monday-Friday, 8am-5pm MST Primary Responsibilities: Responsible for daily scheduling of all physician orders, new referrals, and schedules with appropriate field clinicians Reports new or pertinent patient information to field clinicians Schedules any changes or corrections per physician orders in the home health EMR Ensures new referrals are staffed scheduled correctly with a RN and other field clinicians required to appropriately carry out physician orders Schedules and plots all tasks specific to each patient from the beginning to conclusion of directed patient care: supervisory visits, re-certification visits, discharge visits, transfers, and post-hospital visits Contacts field clinicians regarding any missing or outstanding documentation on a weekly basis Updates clinician schedules frequently to remain in compliance with current physician orders. Communicates all new physician orders and new referrals with contract therapy staff Assists CTMs with contacting field clinicians or relaying information in reference to patient care updates Frequently communicates with management regarding unassigned patient visits to ensure all patients receive care within the timeframe required per physician orders Responsible for entering transfer tasks to appropriate field clinician upon notification of patient hospitalization Prepares weekend staff-reviews, all patient schedules ensuring field clinician is aware of current orders or any changes in their schedules Demonstrates exceptional customer service to all patients, clinicians, and family members Answering of phones with quality customer service Updates patient information into the EMR when changes occur Runs active census weekly- schedules discharges with field clinicians and appropriate tasks to ensure accurate, active census is up to date Responsible for processing discharges/ensuring proper Notice of Medical Non-Coverage (NOMNC) in place before discharge is processed- updating patients schedule to reflect discharge date and notifying medical records to archive patient's active chart Responsible for scheduling per physician order and ensuring Re-Authorization Department has updated order in order to obtain authorization from physician's office Prints out all therapy evaluations from EMR- fills out fax cover sheet with recommendation and faxes to physician's office, ensuring effective communication between physician's office and home health agency Participates with on-call rotation for home health approximately one week per month to include shared holiday coverage Performs other job tasks/duties as assigned by manager You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.
