Overview: The Patient Care Coordinator functions as a team member who, under the direction of the Office Manager, provides support to patients, physicians and multidisciplinary teams to facilitate patient care within the clinic. The patient services coordinator is responsible for maintaining patient satisfaction by providing non-medical patient services, problem solving, training and supporting front desk reception and checkout personnel.SCOPE: This non-exempt position reports to the Office Manager.ESSENTIAL FUNCTIONS: (In compliance with the American with Disabilities Act, ADA, this description identifies the essential components of the job required to be performed with or without reasonable accommodations. Patient InteractionMeets with new patients and assists with coordination of needed prescreening/testing in conjunction with physician orders.Schedules injection classes and/or confirms class appointment with patient as needed by team or office.Provides patient education regarding clarification of physician instructions, medication instruction, and anticipatory guidance under the supervision of the RN.Establishes a compassionate environment by providing excellent customer service, anticipating anxieties, answering questions, providing support.Assures quality care by adhering to Practice standards; follows clinical operations policies and procedures, as well as standards of care set by the state and other governing agency regulations.B. Clinical/Technical DutiesDocuments patient care services by charting in patient and department records including the electronic medical record (EMR). Utilizes the computer in entering labs and printing labels. Completes fee tickets as appropriate.Maintains safe and clean working environment by complying with procedures, rules, and regulations, and calling for assistance from other health care support if needed.Authorized to receive and disseminate lab results from outside laboratories to the patient's RN and/or physician for review and follow up as necessary. Assists the primary clinical teams with daily results as needed. Communicates results to patients via call backs under the supervision of the RN. Utilizes the EMR to provide consistent and timely patient follow up and management of testing and cycles.C. AdministrativeInitiates patient charts and sets up check lists in the EMR.Scans forms, records, and other information as needed into the EMR. •Responsible for generating and maintaining patient prescreening and testing checklists in conjunction with clinical protocols.Communicates with financial coordinator to ensure authorizations obtained prior to testing or starting a cycle. Assists with completing and routing needed insurance forms (i.e., insurance questionnaires). Faxes test results to financial coordinator and/or insurance plan as needed.Utilizes scheduling system for purposes of managing the primary physician's schedule and scheduling patients for the primary physician.Communicates problems to the primary clinical team, i.e. prescreening/testing or financial concerns.Reviews patient records for completion of prescreening inclusive of medical history, appropriate laboratory work, and informed consent.Checks that consents are current and entered into the EMR. Informs patient/couple of consents needing to be updated.Assists in obtaining authorizations for medications. Communicates with insurance plans and pharmacies to ensure patient able to obtain medications in timely manner.Schedules and follows up with patients as necessary.Documents laboratory results in the EMR checklists after review the by RN and/or physician.Documents delivery data in the EMR. Reviews lists for team's patients needing delivery data entry and notifies RN/assists RN with collection.Prepares patient information packets as needed. •Assists team with correspondence and other administrative tasks as needed.Contributes to the upkeep of office equipment and daily operational functions within the office. (i.e. fax, printer, mail, filing, coping).D. Team Communication and InteractionAssists the clinical team to resolve patient problems and needs by utilizing multidisciplinary team strategies.Communicates with other offices if patient to be monitored or have procedure there to ensure accurate information available and needed testing entered as an order in the EMR.Assists in patient retention activities through utilization of the Patient Care Module in the EMR to continually improve services and productivity, increase patient retention, and enhance patient satisfaction.Reliable - reports to work on time, as scheduled, ready to work on a consistent basis.Covers weekends and holidays as assigned.FCI requires all applicants to be fully vaccinated for COVID-19 before commencing employment. New hires will be required to furnish proof of full vaccination prior to their start date.There may be alternative for an approved exemption based on bona fide medical or religious grounds. High School Diploma required, Associate degree preferredMinimum 2 years' experience in a hospital or medical setting preferred.
