Job Responsibilities You will support the Department by utilizing your expertise to fulfill the following responsibilities:•Acts as client advocate in order to coordinate required services or to N/A resolve crisis•Counsels clients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life•Identify environmental impediments to client of patient progress through interviews and review of patient records•Monitors, evaluates and records client progress according to measurable goals described in treatment and care plan•Organizes support groups or counsel family members to assist them in understanding, dealing with, and supporting the client•Performs clinical assessments of the behavioral health needs of consumers presenting for services•Provides case management services related to identified treatment needs•Provides training to direct care staff in implementing behavior modification techniques and treatment plans•Refers client or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education•Receives additional training, as required, to gain full proficiency and experience in all areas•Employee will be part of various outreach events.•Employee will certify on HIV Insti test.•Employee is required to be part of mandatory events such: Augusta Pride, National HIV Day, World AIDS day. Employee will be doing outreach on those events and/or Insti test.This is not a telework position. Minimum Qualifications Bachelor's degree in a related field from an accredited college or university and two years of experience in social service delivery. High School diploma / GED with 4 years of experience is social service.Preferred Qualifications:•Excellent customer-focused interpersonal skills to interact in an effective manner with practitioners, the interdisciplinary health care team, community agencies, patients, and families with diverse opinions, values, and religious and cultural ideals.•Excellent verbal and written communication skills.•Bilingual in Spanish/English, required•Excellent computer skills, experience with electronic medical records a plus.•Experience with CAREWare/HMIS a plus.•Ability to maintain HIPAA compliance, corporate compliance and client confidentiality.Additional Information Employment Information Current State employees are subject to State Personnel Board rules regarding salary.
