Position Summary: The Patient Care Coordinator plays a vital part in administering day to day operations of the hospice department and coordinates the services of all team members. This position works closely with the Clinical Director to meet and promote the standards of quality and contribute to the total organization and philosophy.
- Scheduling out visits requested by Registered Nurses, Social Worker and Chaplains
- Ensuring staff are completing visits in a timely manner
- Serving as the scribe and facilitator for interdisciplinary team meetings
- Gathering necessary information to input a complete referral
- Ensuring that Medicare patients have eligibility and verifying commercial insurance
Essential Duties & Responsibilities:

Other duties as assigned by the Clinical Director. Work Environment This position will perform most duties in an office environment. While performing some of the duties of this position, the employee will travel by automobile and is exposed to changing weather conditions. Physical Demands While performing the duties of this job, the employee is required to see, talk, or hear. The employee is regularly required to stand, walk, sit, reposition patients; use hands and fingers to feel objects, tools, or controls; and reach with hands and arms. The employee frequently lifts and moves up to 20 lbs. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Graduate of an accredited LPN program preferred. Minimum one year nursing experience. Two years home care or hospice experience preferable Current LPN license in the State of Florida. Current CPR certification. Current driver's license and valid auto insurance.
