This full-time position will primarily work in Brainerd, providing Care Coordination to clients across multiple Northern Pines Services and all Service Locations.Essential Functions include:Attend Integrated Treatment Plan meetings for clients enrolled in Care CoordinationAttend Rule 79 Case Consultation Meetings to facilitate client needsAttend Thursday Integrated Consultation MeetingsAssess clients need to establish Primary Care (Medical), Dental Care and Eye Care. Provide needed linkage and follow-up to linkageConnect with clients to address identified needs on the SDoHProvide resource and referral that would meet identified needsClose that loop on resources and referrals to ensure that client has received the resources and/or engaged in the referred servicesConnect with client to discuss services that are recommended by Mental Health Professional following a comprehensive evaluation and refer to services client prefersAdvocate for clients to help ensure their needs are metAssess client satisfaction with servicesProvide MNSURE Navigation to clients without insuranceProvide Crisis Resources to clients and assist in connecting to Crisis ServicesProvide Health and Wellness Education and Information to clientsSchedule and attend on-going visits for clients enrolled in Care Coordination Program.
