You'll play a vital role in supporting patients and their families through effective coordination and administrative tasks. Responsibilities:
- Collaborate in coordinating patient care plans under supervision.
- Assist in scheduling home visits and ensuring timely care services.
- Communicate effectively with healthcare professionals, patients, and families.
- Maintain accurate records of patient care plans and modifications.
- Support patient and family education on available resources and services.

- Willingness to travel within the designated territory to various facilities.
- Proficient in Microsoft Suite and basic computer knowledge.

Daily Tasks Include:
- Engaging with office managers, physicians, and patients.
- Documenting and promoting the company's services.
- Traveling to different facilities to represent services.
- Generating referrals and aiding facilities with their needs.
- Identifying patients requiring additional care and obtaining approvals.
