POSITION SUMMARYThe Patient Care Coordinator (PCC) will be responsible for ensuring that patients receive comprehensive,coordinated care throughout their healthcare journey. The PCC will work closely with patients, theirfamilies, healthcare providers, and other stakeholders to facilitate communication, manage care plans,and provide necessary resources, referrals, communication, behavioral health, coordinated care, andinsurance.Essential Functions Statement(s)* - Patient Support and Coordination* - Develop and manage personalized care plans for patients, addressing their unique needs andpreferences.* -Serve as the primary point of contact for patients and their families, providing guidance andsupport throughout their care journey.* -Facilitate communication between patients, healthcare providers, and other stakeholders toensure timely and accurate information sharing.* -Care Plan Management* -Monitor patient progress and adherence to care plans, making adjustments as necessary toensure optimal outcomes.* -Coordinate referrals to specialist, behavioral health services, and community resources.* -Assists patients with scheduling appointments, transportation, and other logistical needs.* -Identify and address care coordination errors and gaps, working to continuously improveprocesses and patient outcomes.* -Training and Development* -Participate in ongoing training in customer service, referrals, quality,communication/transportation, behavioral health, coordinated care, and insurance. ------Stay current with best practices and advancements in patient care coordination.* -Resource Management* -Connect patients with appropriate insurance, financial assistance programs, and communityresources.* -Provide educational materials and resources to patients and their families, empowering themto make informed decisions about their care.* -Data Management* -Maintain accurate and up-to-date patient records and documentation.* -Other duties as assigned.SKILLS & ABILITIESEducation: High School Diploma or General Education Degree (GED)Experience: Experience in customer service, healthcare, or related field required.Computer Skills: Use of electronic medical record and other information technology will be required.Company Paid Benefits:* Health, dental, and vision insurance* Paid time off (sick and vacation, up to 96 hours accrued annually)* Eight (8) paid holidays* One (1) floating holiday* One (1) birthday holiday* Life insurance* Long-term disability* Free ARcare clinic benefits for employees with ARcare health insurance* Retirement plan (401(k) with company matching up to 200% of 4% after 1 year of employment)* Additional supplemental and family coverage availableApplication Assistance:If you need assistance with the application process, please email ...@arcare.net.Equal Opportunity Employer:ARcare is committed to creating a diverse environment and is proud to be an Equal Opportunity Employer.
