Job Title: Patient Care Coordinator / Patient Services Representative

Job Type: Full-Time (Monday to Friday, 8:00 am to 5:00 pm)

Compensation: $18-$22 per hour depending on experience

Position Overview:

Do you have a passion for supporting patients and making a difference in their care journey? As a Patient Care Coordinator with Summit Home Care and Home Health Care, Inc., you'll play a vital role in ensuring a seamless experience for patients and their families. This position involves coordinating care by collecting, organizing, and tracking patient information and documentation, providing essential administrative support that ensures our clients receive timely, effective care. This is an ideal role for individuals with a background in healthcare customer service, particularly in fast-paced settings.

Patient Care Coordinator Responsibilities:

  • Manage Documentation: Track and send essential face-to-face documentation for physician signatures
  • Collect Client Information: Gather important documentation for hospitalized clients, such as History & Physical (H&P), Resumption of Care (ROC), and physician orders
  • Conduct Client Calls: Perform pre-admit calls to verify client information, along with follow-up calls at five-day and discharge intervals
  • Track Hospitalized Clients: Monitor hospitalized clients and inform Community Liaisons of discharge updates for a smooth transition back to care

Qualifications:

  • Education: High school diploma required; college degree preferred
  • Medical Knowledge: Knowledge of medical terminology preferred
  • Experience: At least one year in an office or healthcare setting, ideally in a fast-paced environment
  • Skills: Strong verbal, written, and telephonic communication skills; demonstrated typing speed of 50 words per minute or more
