PATIENT CARE SUPPORT COORDINATOR (REMOTE)*MUST BE LOCATED IN NY STATE*Qualifications:+ Previous healthcare experience+ Call Center experience or related+ Experience with managing performance metrics+ Customer Service Skills+ Knowledgeable in Microsoft Office Suite+ Minimum High School Diploma+ Must have quiet space to work and good internetJob Overview:This individual would be making calls to our members to verify their home care services.+ Provides outreach to members via phone to support with care plan next steps community or health plan resources questions or concerns related to scheduling and ongoing education for both the member and provider throughout care/service+ Provides support to members to connect them to other health plan and community resources to ensure they are receiving high-quality customer care/service+ May apply working knowledge of assigned health plans activities and resources+ Serves as the front-line support on various member and/or provider inquiries requests or concerns which may include explaining care plan procedures and protocols+ Supports member onboarding and day-to-day administrative duties including sending out welcome letters related correspondence and program educational materials to assist in the facilitation of a successful member/provider relationshipWork Environment:+ Monday-Friday -8:30AM-5PM EST+ REMOTE-WORK FROM HOME- equipment providedPay and BenefitsThe pay range for this position is $19.00 - $19.00- Medical, dental & vision- Critical Illness, Accident, and Hospital- 401(k) Retirement Plan - Pre-tax and Roth post-tax contributions available- Life Insurance (Voluntary Life & AD&D for the employee and dependents)- Short and long-term disability- Health Spending Account (HSA)- Transportation benefits- Employee Assistance Program- Time Off/Leave (PTO, Vacation or Sick Leave)Workplace TypeThis is a fully remote position.Application DeadlineThis position will be accepting applications until Dec 18, 2024.About TEKsystems:We're partners in transformation.
