Nuvance Health has a network of convenient hospital and outpatient locations - Danbury Hospital, New Milford Hospital, Norwalk Hospital and Sharon Hospital in Connecticut, and Northern Dutchess Hospital, Putnam Hospital Center and Vassar Brothers Medical Center in New York. Summary: This Per-Diem Role is for Patient Care Technician II PDP. Performs patient care activities and non-professional services. These include addressing the personal, physical and emotional needs of the neonate, pediatric, adolescent, adult, or geriatric patient and family. Practice must be consistent with the philosophy, goals, policies and procedures of the Nursing Department and Medical Center administration. Population served based on scope of services in the department. Responsibilities: 1.Assists in the delivery of patient care by performing, specific patient care tasks per scope of service and as directed by the Registered Nurse. 2.Assists with admissions, transfers, discharges and transport of patients. 3.Obtains and records Temperature, Pulse, Respirations, Blood Pressure, Oxygen saturation, weights, and Blood Glucose testing. Reports abnormalities and condition changes to primary nurse promptly. 4.Demonstrates competence in advanced skills to include, but not limited to: oral suctioning, enemas (no Fleets enemas), clean dressings after a RN assessment, tube feeding set-up, external catheter (condom catheter), post-mortem care and suction canister set-up. 5.Communicates effectively and frequently with health care team members to share information and solve problems in a positive and result-oriented manner both verbally and electronically. a. Documents legibly on all hospital forms using standardized abbreviations. b.Gets patient report from the RN and off-going PCT at the beginning of the shift. Gives end-of-shift report to the primary nurse and oncoming PCT at the end of the shift. c.Participates in walking rounds with the oncoming and off going PCT at shift change. 6. Provides personalized care to patients and families. Identifies and documents special needs on the patient rounding sheet. 7.Utilizes available educational materials, programs, nursing literature and resource personnel for career development and attends mandatory in-services to maintain unit competencies. 8. Utilizes material, equipment and time in a safe, therapeutic and cost-effective manner. 9.Assists with appropriate tasks in all code situations (i.e. CPR, obtaining equipment, etc.) under the direction of the Code Blue Team. 10.Adheres to Organizational infection control practices and policies and to established safety and security policies, including using two (2) identifiers whenever taking blood samples or performing any procedure. 11.Assists with maintaining the general cleanliness and appearance of areas on the unit, including, but not limited to: a.Cleans treatment rooms, showers, commodes and IV poles after use. b.Cleans kitchens (refrigerators, ice machines, sinks, counters) as assigned. c.Cleans counters and empties trash in nursing station as requested. d.Cleans other areas such as storage areas, linen rooms, staff lounge as requested. e.Cleans, restocks and maintains patient care supplies. 12.Maintain and Model REACH Values (Respect, Excellence, Accountability, Compassion, Honor). 13.Demonstrates regular, reliable and predictable attendance. 14.Performs other duties as required. Education and Experience Requirements: • High school diploma or equivalent. • Current NYS Certified Nurse Aide Training certificate or have successfully completed the first clinical rotation at an accredited school of nursing. • AHA BLS provider certification course required • Three (3) to six (6) months of CNA or PCT experience preferred Minimum Knowledge, Skills and Abilities Requirements: • General knowledge of high school subject matter including sciences, simple medical procedures / terminology, and office equipment. License, Registration, or Certification Requirements: • Current NYS C.N.A. certification or completion of first clinical rotation at an accredited school of nursing • BLS certification is required 90 days' from date of hire Location: Vassar Brothers Medical Center Work Type: Per Diem Standard Hours: 0.01 FTE: 0.010000 Work Schedule: Evening 8 Work Shift: 3p-1130p Per Diem, a minimum of two (2) weekend shifts of eight (8) hours or more per twenty-eight (28) day staffing cycle and two holidays. Org Unit: 1226 Department: Med Surg 6 - Neuro Exempt: No Grade: U2 Salary Range: $21.18 Hourly Working conditions: Essential: * Significant manual skills / motor coord & finger dexterity * Significant occupational risk * Very Heavy effort.
