As a member of the interdisciplinary team, the PCT gives direct care to patients, under the supervision of the nurse. The PCT is responsible for a variety of tasks and services to assist in the provision of safe care, and comfort for patients.

ResponsibilitiesClinical Excellence/Patient Care
· Assists patients with tasks they are unable to do for themselves, (ADL's, feeding, grooming, mobility, etc).
· Monitors the health of patients via vital signs, glucose assessments, safety rounds, and reports as appropriate to the nurse.
· Observes patient's condition, and any changes in their physical, emotional or mental status, and communicates the status of the patient, including any changes, periodically, to a supervisor, case manager, or nurse.
· Promptly completes documentation to assure an accurate legal record of patient's care.
· Follows infection prevention measures.
Environment
· Assists the team to ensure adequate stock and supply is available for patient care.
· Supports the team in keeping patient rooms and the work area safe and clean.
Clerical
· Assists the team to respond to patient calls in a timely fashion.
· Supports the medical team in handling paperwork.

Education:
BCLS required within 180 days of employment
Certified Nursing Assistance certification
On-the-Job training or sufficient previous experience if assigned to complex unit
High School Diploma preferred
Experience:
Prior CNA experience in acute or skilled environment
License(s)/Certification(s):
Certified Nursing Assistance certification, or the equivalent education, ie; EMT, etc
Knowledge/Skills/Abilities:
Writes, reads, comprehends and speaks fluent English.
Multicultural sensitivity.
Microsoft Office – basic computer skills.
Customer/patient focused.
Critical thinking skills using independent judgment in making decisions.
Other:
